Straddle your whumpee, pinning them down while you administer first aid.
Pain, not so intense they can't function, can appear to be alright, but pain such that they're distracted.
Their memory is terrible and they don't know why. They can't focus on anything. They have to keep moving, they can't hold still.
They know they're hurt, yes, but they can keep moving, so what's the issue?
Only hours later dawning on them that being in pain is taking up so much brain space they can't do anything else.
"Will you hold me down for this?"
"Yeah, I can hold you."
"No, I need you to hold me down."
Whumpee in pain and unable to get themself to safety and help, looks up hopefully when they hear a noise. Only, it's not a friend standing there, it's an enemy.
When the defiant whumpee flinches despite their brave face, the way they sag in defeat after whumper leaves. Despite their attitude, they can't do shit to save themself but they only let the fear show when whumper is gone.

Lonely? That's because you're locked away awaiting rescue.
This microwave meal? You should be grateful your captors gave you one at all.
Where is your pain, your hurt, your fear? Inflict it on your characters and watch them struggle.
After the horrors, whumpee is alone in their pain, flailing and sobbing as they try to help themself or find help.
The anxiety that comes before the whump. When whumpee knows they have to put themself in a situation, and they know it's going to be bad. What is on the line that is so important that whumpee is doing this to themself?
