Firstly, let’s have a disclaimer: both Winchesters are dangerously codependent and have made extremely poor choices because they didn’t want to lose each other.
At that point, end of season 4, Dean (and probably the audience) had no way of knowing whether what Sam had done had had a permanent effect on him. Iirc Dean expresses a belief that letting Sam have demon blood would turn him into a monster and he’d have to kill Sam. Thus, the better choice in Dean’s opinion was potentially allowing his brother to die because he’d be “dying human.” And of course Dean would not have to strike the final blow himself.
Dean’s idea of love is not letting someone he loves turn into a monster.
Dean’s idea of love also includes a host of abusive tactics and behaviors that under no circumstances should be practiced in a real relationship. The canonical relationship between Sam and Dean inspires me to write novel-length fanfics, but make no mistake, there are some extremely unhealthy elements in it. So, leaving an addict to die may not be love, but understand that Sam and Dean do a lot of stupid stuff out of their unhealthy need for each other--for Dean it is Sam alive and human, for Sam it is his big brother’s approval--and if you don’t like their decisions you’re probably right. XD
