If you're looking for health coverage, then it might be time to take a look at the Empire Plan.

The Empire plan was developed by a group of doctors, nurses, administrators, brokers and other professionals. The Empire plan was developed for those that want to provide quality health benefits to their family members, but can't because of the cost. This is why the Empire plan was developed. It is like a “group of one” where everyone pays a small monthly premium to insure that they get the quality and care that they need.

One of the things that makes the Empire plan so unique is the paperwork that they provide to their members. This is called the Empire Benefits Summary. You can find the benefits summary on the front page of the website. This provides the following detailed information on the various plans that are available. There are several different types of plans and they include; SIP (Supplemental Insurance Program) which is self-explanatory, HMO (Health Maintenance Organization) which is managed care and is usually provided through a private insurance agent, PPO (Preferred Provider Organization) which requires a referral from a primary care physician and is managed care, and POS (Point of Service) which is just what it sounds like. This plan also includes information on how the plan will work and provides details about the different plans available and the benefits that are provided.

Other benefits from the Empire plan are the following; the participant is required to choose a network that accepts their insurance and pays the monthly premiums. The participant will have to pay the non-network deductible for the basic medical program. There is also a fee-for-service benefit wherein the patient will need to pay a co-pay when going to see a doctor, specialist, or psychologist. The mental health and substance abuse program is designed to help with; rehabilitating the patient, treating the illness and family support. The family support is designed to give the patient the tools to live an independent life and to handle the illness.

The Empire plan prescription drug benefit contains the list of participating pharmacies along with all the information necessary to calculate the discount rates for the user. This includes the PPO, POS and CVs caremark. There are also special discount rates for students and employees of healthcare facilities. There is also a provision for the transitional care program for former patients of certain hospitals and nursing homes in the Empire State Plan. The transitional care program helps them receive necessary medications and continues coverage for prescription drugs while they are undergoing treatment in a different hospital.

It is important for the patient to read the complete and detailed summary of the benefits, limitations, exclusions and conditions before enrolling in the plan. This is especially important for the student employee as this is a unique opportunity to gain access to many benefits that their employer may not be offering. This summary should include the following general information; patient's name, address, insurance type, copy of the ICD-9 diagnosis code(s), if the drug plan covers major tranquilizers, anti-anxiety or sedative drugs and the cost of each drug per month. It should also include the patient's payment options, provider, amount of cash payments, prescription drug coverage amounts and details on their eligibility for the discount rates.
