I had a lot of feelings last week about transfem!JTK even tho I headcanon them as a very repressed gay man with enough internalised homophobia to sustaine a mega church (normally) but yeah, when nothing is canon everything is. And creepypasta is a massive sandbox and you are supposed to play around in it :)

Her just knowing her parents wouldn't respect her being trans
Her also having a lot of internalised transphobia from her upbringing and bullying
Her being more upset if her bullys called her "gay" then when being called a girl or girly even though she thought (some) guys were hot
Her cutting a Glasgow smile was a very bad attempt at dealing with her gender dysphoria
Not that she knew it was gender dysphoria, she always just thought she needed a little fixing, something was wrong maybe she needed to be a bit more pretty
Someone at the mansion casually mentions trans people existing and she couldn't let that thought go
It hounted her for a while and her mental health spiraled. More killings, way more brutal than usual at that, more fights with other proxys, she turned more and more irritable than she already was. She was a hungover or drunk mess for two months
To break the egg she had to had a complete mental breakdown after she wandered around the woods for a few days. The thoughts wouldn't go away, the feeling of wrongness would leave her, then she broke down and realised she's a woman. It wasn't an easy realisation
(some other proxy had to look for her and carry her back because she went catatonic for a bit)
It still took a while until she came out, she started breaking into pharmacys, stole estrogen and injected herself. When the changes were visible she wandered into the living room showing off her new tits in a tight top and declared that everyone remembered her name wrong. It has always been Joan!
She models her voice and hair after Joan Jett (hence the name, (listen to her music btw)) she always thought that bitch was cool and she doesn't have to change her JtK thing
Everyone who misgenders her will get injured (proxys) or killed (everyone else)
The proxys are surprisingly good with her pronouns, there are no incidents. She expected otherwise
She might have decided one day to cut her dick off and and EJ might had to give her an emergency vaginoplasty (while looking up how the fuck to do that on wikihow)
She is very weird about her femininity at first, has to be seen as sexy and very fem before she can accept herself as who she is
A lot of muscles, not very conventionally attractive (esp the scars), she will however claim the opposite
I can't stress enough how weird she is about her femininity at first, she needs a few years until she starts wearing pants again. A lot of internalised misogyny and transphobia to work through. She would only wear skirts at first and drench her scars in lipstick (she had Nina as a teacher of makeup tho, eventually she figures it out) acceptance isn't liking or being happy with it. Getting there took her way longer
"Go to sleep" she voice trained on that phrase. It's higher pitched and more animated than before but she still has a raspy, threatening voice
She still can't shake the feeling of "betrayal" of not being Liu's brother anymore
She talked about it crypticly to Liu a lot when they were children. None of them understood what was going on. Ej at some point started to be her doctor, he hates it because she is reckless! He monitors her results and wants to puke as soon as she starts talking about injecting the highest save dose of E into her arm right away. Does she disinfect the spot at any time? Hell no! Her hormones are all over the place.
He is close to begging her to just let him inject the E into her but she won't because she likes it; but she let him adjust the dosage. For some mysterious reason her transition goes smoother after that.
She cut out letters from a newspaper and wrote a letter to the fbi to let them know that her name is Joan, stop misgendering me!! It was a threat in a letter.
One police officer called her him and she hunted him down. They had to scrape his family from the ceiling
