Provider Self-Assessment Deadline Fast Approaching!

 As part of the state’s approved Transition Plan, all residential, day, and ISC organizations should be working to finalize and submit the required self-assessments.  As a reminder, all assessments are due to be completed and entered into Wufoo by March 31, 2015.  Regional teams are reaching out to providers during the next few weeks to ensure all providers are working towards having assessments completed and submitted on time.  Some providers have noted that Wufoo will not accept large attachments.  If you experience this problem please email the information to your regional compliance director.
