Posterior Tibial Tendon Dysfunction occurs when said tendon--usually connecting the Posterior Tibial Muscle to parts of the arch of the foot, maintaining arch height, and preventing Pronation--becomes swollen, inflamed, and pained, resulting in Tendonitis and Dysfunction. Early treatment of the disease is recommended, because the condition is progressive, and ignoring it may lead to greater damage to the foot.
Untreated, the disease may cause further flattening of, and pain outside of the foot. Braces are among several non-surgical treatments used to treat the condition, the alternatives being Orthotic devices, immobilization, physical therapy including ultrasound, anti-inflammatory drugs, and changes to one's shoes. Surgery tends to be used when less-invasive measures fail. U.C.L.A's Foot and Ankle Institute recommend early treatment, and say that Orthotic treatments are quite effective at solving the problem. The study stated that braces "...aid in a more functional gait," implying that braces are more palliative treatments, better used as compliments to other methods.
The study stated that a torn tendon will, most likely, require surgery, and that failure to receive such treatment may result in further arch collapse, and arthritis. This condition may be caused by an extraneous bone in the foot that becomes detached--in that case, the bone will often have to be removed. It suggests a new method of treating tibial tears--connecting half of an adjacent tendon to the torn tendon--but whether this can be used as a substitute for removal of the bone is unclear. Douglas H. Richie, Jr. Doctor of Podiatric Medicine, and contributor to the website PodiatryNetwork, says that the condition is most likely in the obese, diabetics, and older women. He recommends insuring that all non-surgical options have failed before considering surgery, and awareness of the long debilitating recovery from surgery. Fifty-two percent of victims are diabetic, obese, or have high blood pressure.
Current classifications may be too broad, and the disease might be better categorized if the current categories were split into multiples subgroups. Surgery seems to have a significant rate of long-term success: Two studies followed a total of 27 patients for years after their surgeries, and the subjects experienced neither deformities, nor arch loss. Their success, however, may have resulted from the lack of a functioning peroneus brevis. One study concluded, "The development of a flatfoot in tibialis posterior tendon dysfunction is therefore unlikely to be the result of lack of ‘sling’ support of the medial longitudinal arch from the tibialis posterior tendon only." These studies focus on a very small number of patients; however, we cannot be sure how accurately they reflect the entire spectrum of victims.
One website suggests Orthotic devices, such as braces, for Stage I-Tendonitis without tendon tear--surgery for Stage II--tendonitis with partial tendon tear--and the fusing of joints for stage III, Tendonitis with full tendon tear. Based on the current evidence, a patient would be wise to start treating the disease with Orthotic devices such as braces, and to do so quickly, as the disease is degenerative, and painful. Surgery, though unpleasant, is quite useful, but should be considered later if the case is severe. Techniques for treating this disease, including braces, seem to be advancing, so younger people who do not have the disease, but fear suffering it, do have hope for the future.

Silt Barriers
In the wake of increasing pollution, the US government passed bills known the Clean Water Act [1998] and NPDES phase II [National Pollution Discharge Elimination System] Regulations enforcing the protection of water resources. According to these acts, the states are required to identify all water bodies, where required pollution controls are not enough to maintain water quality and standards ensuring to include the uses of the water and severity of the pollution problems.
The organizations, using water bodies as a dumping ground, ensure that content they release is safe and not too much of it is dumped in a certain area. Hence, most constructions near water bodies are required to use silt barriers in order to meet the rules and regulation set by the respective administrations.
Silt barriers are a temporary sediment control devices installed during construction to control the soil from running into the water bodies. This is an effort by the companies to protect the water quality. Silt barriers are used as perimeter controls to collect the silt in place when it gets mixed up in water as a result of the construction works. Silt barriers are usually there to control the offsite transport of spilled oil as well.
The silt barriers are of several types and are used in several ways. First anchoring posts construct a silt barrier at regular intervals, no more than 6 feet, with a length of filter fabric stretched between them. The placement of the posts is usually downslope or low areas, or areas where storm water mostly runs down. The filter fabric is embedded or fixed securely on the ground between the supporting poles.
Efforts should be made that the silt barrier be located five to seven feet beyond the base of disturbed slopes. Otherwise, overflowing may occur and will result in fence failure. It is also that if the barrier is too short, 16 inches or less, it will be weak before overflowing and will require clean outs more often. That is why it must be ensured that the barriers are standing up to 24 inches to 34 inches to give better results. It is very important to make sure the poles are well supported to prevent hazardous accidents.
The requirement for the fabric is a pervious sheet of nylon, polyester, propylene or ethylene yarn certified by the manufacturer, but safe for health. Synthetic fiber fabric must have ultraviolet ray inhibitors and stabilizers and must be useable for at least six months. Wooden stakes must be five feet high minimum manufactured from sturdy woods such as oak with diameter 2 inches, and pine with diameter 4 inches. In case steel stakes are used, they must have a minimum weight of 1.33 pound per linear foot and a height of five feet.
The major reasons that the effectiveness of silt barriers in field studies has generally displayed poor results are poor maintenance, improper installation and bad location. All in all, if installed properly, checked frequently and maintained regularly, silt barriers can be effectively used to contain the silt washing off the site during heavy rainfall.
