Pros:

Little brother will have a loving hand on him that's focused on his pleasure and his pleasure alone.

Itachi unconditionally loves him and does not care how many partners Sasuke has had before him, and he does not consider Sasuke dirty, knowing how offensive and damaging the notion is.

Itachi will not mind if Sasuke is also taking clients during the same timeframe they've become intimate, Sasuke does not belong to him, and it is up to Sasuke to quit his sex work when he deems it fitting and proper, not Itachi.

Cons:

Sasuke will likely come home battered and Itachi will gently refuse intimacy in favor of tending to his wounds.

People have been killed in sex work, so there is the possibility of Sasuke not coming home.

Sasuke already considers himself dirty and unworthy, so he won't show or tell Itachi how he truly wants to be pleased.

Incest but it bothers neither of them, it's simply a social taboo.
