The spirits that follow him all function as death. He gets on the bus to get away, sees his son and follows him because that's his will to live; his son. The son has that robot arm because this is all taking place in dude's mind, and he subconsciously wants his son's arm back so that's how he's represented in this dream sequence. Anyways as they walk under the bridge dude's dream gets cut off, his time is up, which is why he drops to his knees. Turns out the image of his son was death as well, the boy pulls the uwop, pops is dead. Basically the point of his son ever being in the dream was a false sense of hope/warmth while pops endured the cold embrace of death, which is signified by the eerie world pops had to traverse to try to escape. The image of his son embodies his determination to try to survive for the sake of his son, though it was all just a futile dream in the end. Shit is real cool.
