this might b a dumb question but how do you fail a mission in dragon age

I'm actually not sure about most of those. I've never seen any of them in my game aside from the generic "you died" one (the first one in red). I only found them accidentally while snooping around in the game's textures.

Speculation based on stuff I've read:

According to the notes on my post, someone says you get the "Envy victorious" one if you die in the Fade during Champions of the Just

According to the Prima guide, you get the court approval one if your approval rating drops too low during Wicked Eyes and Wicked Hearts

The templar being killed in the Red Templars version looks like Ser Barris... I actually know nothing about the Champions of the Just quest because I've only sided with the mages so far, but I just noticed that
