I actually at one point completely (possibly over)analyzed the fangame SickMind and I developed my own headcanons on what everything meant. It's been a while and I might have to review and change some headcanons about the npcs but I do remember one of the headcanons I had was that the npc with the IV drip stand walking around the hospital had Cotards delusion which was why she looked so different, it was symbolism for what she thought she looked like. I dunno how I even came up with that but that was one of them.

You know what I really want?

A Yume Nikki style game where you, after exploring a certain amount of the dream world or collecting a certain number of effects, wake up and go outside. After a brief moment you return inside and go to sleep, only to find the dream world is different. Not everything's changed, but since some things do that implies that things are going on in the real world with this character.

Things like a previously bright and friendly area becoming dark and full of enemies, a previously non-caring character starting to follow you around, new areas appearing and old ones disappearing, etc.
