It's simply a genjutsu. Whatever injuries the other receives are gone as soon as it's dispelled, which is as soon as Itachi is a safe distance away, for now. Anyone else might've attacked by now-- but Itachi also had endless patience.

He doesn't answer to the threat of him hurting Sasuke. He's protective, yes, but he knows where to step in, and when to stay back. The other doesn't even know where he is.

Had he been any sort of a braggart, or proud in his abilities, he might have been somewhat honored that a God had taken such a ....strange liking to him. Even if it was attempting to cause him bodily harm. What use does a God have with him? He doesn't follow any religion, and he's a pacifist by nature. He's not about to convert to something that requires him to kill people. He has no desire for immortality.
