kotaro either was the one who ran over sakura with the truck or was close to her. proof: he was holding sakura's audition folder that she had when she was ran over. so either he was the culprit or he was close enough (either in proximity or in personal relationship) to be allowed to retrieve the file after the accident. another alternative: grave robbing (unlikely though. she won't be buried with it)

points in favour of him being the culprit: his strong motivation for reviving her even though she's not as conventionally "legendary" as the others, his terrible driving skills and the fact that he owns a similar looking van. points against this theory: students of sakura + young kotaro's age probably aren't legally allowed to drive. counterpoint: illegal driving.

close theory: if he was physically close to the accident, his guilt about it makes sense, since he blames himself for not being able to prevent it. if they had a close relationship, it also explains his strong feelings and motivation for reviving her.
