If Services for the Underserved provides federally funded housing for about 37,000 New Yorkers, can the Department of Health and Mental Hygiene be compromised in its ability to manage institutional abuse by service providers when the fiscal incentives for liability containment are substantial?

06/02 & 06/04/25: I sent my Grievance Complaint to multiple channels of Oversight email addresses. -- Pure Crickets, ghostmode.
06/06/25: Despite having acute elevation in both Creatine levels and White Blood Cells, my request for advocacy funneled me into an interaction with Psych, then Isabelle Hashim and Maria DePena-Nowak declared a psych-hold required. Although I had a laminated 88-page double-sided documentation of chronological archive of institutional abuse, I didn't stand a chance of being helped.
06/28/25: Why would NYP send their Mobile Crisis Team to my residence three consecutive days and leave an 8/8/25 PCP appointment on my door? I did not miss any appointment, as I verbally declined outpatient treatment with Athena Clinic, when they called to reschedule. I only agreed to the appointment because SW Angelina Hofstede said I had to sign appt documents to be released from the psychward; but if I chose not to go....
07/14/25: Why would New York-Presbyterian ignore my request for discharge paperwork from their Medicaid Farrell Clinic for over six months? I had to disenroll from Medicaid for existential concerns.
07/28/25: Why would HPD request nonessential documents and, even after receiving them, issue a subsidy Pre-termination letter?
08/11/225: Why would SUS complete a wellness check, by instructing the front desk to buzz my intercom, and refuse to state... What agency issued the wellness check? Particularly because no one had consent, not even SUS, to be in any form of correspondence regarding my personhood.
08/21/25: Why would Nefta Sterling, Manager of Compliance at Concord Management of New York withhold and refuse my lease renewal. Then inform me that I didn't have a lease for the prior two years, when in fact I did? What exactly is going on with their documentation? Audit Flag.
08/22/25: Why would HPD have mold complaints listed among the concerns addressed by its agency, but when HPD inspected my apartment, I was told they do not test for mold?
10/04/25: Why would Concord Management of New York, whose building manager, Rocio Frias, has stonewalled me since I declined illegal floor repair in August of 2024, contact HPD and request that they complete an unannounced inspection under unstated reasons on October 4th, 2025?
10/22/25: Why would I receive a backdated, baseless HPD-issued abatement letter that was inoperable?
11/17/25: Why would SUS peer specialist Nakida Marryshow email me, stating that DOHMH informed SUS that I was considering relocation, and share a link to Housing Connect, whilst CC'ing the three employees whom my grievance complaint named as harmdoers?
12/20/25: Why would NYPD knock on my door at 10:46pm on a Saturday? Initially, a noise complaint was the cause for their suspicious appearance, which was improbable because my volume was moderate and it wasn't yet quiet hours. However, when I corrected them, the narrative changed to an Emotionally Disturbed Person concern. Additionally, I disenrolled from Medicaid about two days prior. The timing was curious.
12/23/25: After I queried the DOHMH information shared with SUS regarding their fabricated narrative of my relocation consideration, I named Gail Wolsk in several ignored or deflected emails that were cc'd to all five agencies. I then received a letter from Gail Wolsk in the mail telling me to, essentially, call 311 and run in a death spiral? Whilst omitting that DOHMH engaged in interagency discussions regarding my complaint, as early as June 17th of 2025, Gail neither contacted me nor acknowledged receipt of my grievance complaint before she mailed the letter.
1/28/26: Why would Bianca Lopez, a Tenant Coordinator for Community League of the Heights, inform me that I must remove my door bolt for the backdated, baseless HPD-issued abatement letter to be possibly reviewed?
04/09/26: Why would I request the 2026 Recertification Package from Concord Management of NY after they didn't attach it in the email confirming my income documents had been received, and instead, I acquired an HPD recertification for a tenant living in Unit 3C? The package was not only past due; it was blank, but it had the tenant's name, Social Security number, and his signature. The packet was in English. This individual is a Spanish speaker. Why would Concord have such documentation for HPD, and why would I gain possession of this HIPAA violation? I have yet to receive my 2026 Recertification package from Concord Management of New York.
06/11/26: My grievance complaint remains unaddressed. I fear the system is behaving reflexively within baseline dysfunction.
This is warfare. I was under siege. I await evac.
