Naruto had yet to tell Sasuke how he felt since VOTE2: he had wanted to, but there was never a good time to do so. Sasuke was wrecked from guilt for his crimes and spent all his time on missions trying to atone for it and to redeem himself in the eyes of the villagers, so he barely spent any time with Naruto post-war. Naruto didn't pressure Sasuke to stay in Konoha because he wanted to respect Sasuke's wishes of atonement. However Sasuke interpreted this as, Naruto not caring much about him - that Naruto had only chased him for so long because Konoha NEEDED his powers and not out of any kind of platonic or romantic feelings. So Sasuke thought, once he was back to serving Konoha, Naruto no longer "cared." Sasuke spends his final years lonely and in extreme anguish because some people still have not forgiven him and see him as an irredeemable monster. Sasuke yearns for Naruto and has thoughts of coming clean about how he feels, but doubts Naruto feels the same way or that anyone could possibly love him after all the things he did.

Sasuke and Naruto's last interaction before he fell sick was also tense and left Sasuke feeling that his feelings toward Naruto were one sided.

Tsunade and Sakura conclude there is no cure to Sasuke's sickness and he won't survive; so Naruto takes Sasuke back to the Uchiha complex where he can be comfortable in his bedroom rather than leaving him in the hospital.

Naruto breaks down. He stays by Sasuke's side and cries basically telling him everything and how much he loved and continues to love him, that it hurt him how Sasuke felt the need to "atone" to begin with, and that he wished they had spent more time together. Sasuke hears, but says nothing. When Sasuke has some energy he silently cries in pain because he feels like he is stuck in a limbo between alive/dead. Naruto doesn't know how else to comfort him other than holding him, gentle touches, and telling him how he avenged his clan and redeemed himself, which soothes him. Sasuke's body eventually cannot take it anymore, but Naruto's presence gave him comfort and made him feel loved/appreciated.
