The image is communicating the idea of medicine and where it can get us, this woman has more chromosomes of a man and although her chances of getting pregnant and having children where very slim she underwent a treatment and made it happen.

Barthes describes the concept of time and how people and things change. He remember his mother a certain way and as he looks through the pictures he comes to realize that he remembers his mom almost as a child. He goes on to saying how a photograph can depict a certain thing but can mean or appeal to an individual in different ways just as he had no real connection until the photograph of his mother as a young child. Pictures can hold a lot.
