Physician and trainee suicide is a terrible consequence of a culture and system that hurts everyone and leaves deep cuts that run throughout the country. Our perspective (or lack thereof) on wellness in medicine means we give worse care to patients because we are burnt out, because we are unsupported, and because when we point at the problem we get punished. This cultural de-emphasis on true wellness is pervasive and extends everywhere in medicine – it even means that we are culturally discouraged from speaking out against awful things happening to patients in hospitals, and it even means we are less likely to promote wellness in patient care and education. Medicine is always going to be a challenging profession, and that’s a big part of the reason I chose it, but it should also be rewarding and fulfilling. If I’m gonna work this many hours a week, I better be participating in a system I feel is largely benefiting people and not fighting against one that values the wrong things and is always working against me and patients. We have a lot of work to do.
During my time in med school alone, there were two suicides. One day recently, my third year med student was not his usual self and I pulled him aside. It turned out that one of his classmates had been found dead earlier that morning, from what appeared to be a suicide. He came to work anyways. He had not planned on discussing the matter at all. I offered to dismiss him immediately, and he said he didn’t want to look weak, he wanted to be helpful. It killed me inside, that the culture of our profession would sooner undermine our wellbeing before admitting that we are but humans with human emotions. This story isn’t new. It isn’t dramatic. It’s how it goes when a colleague passes. These struggles and frustrations and confusion are all too real. We ALL must do better. Physicians are killing themselves faster than any natural causes of death are taking them.
