Long COVID's brain fog finally has a suspect lineup: overactive immune cells in the brain, a leaky blood-brain barrier, and inflammation that refuses to clock out even after the virus is long gone.
A new expert review in Translational Psychiatry pulls together the evidence that chronic neuroinflammation drives the cognitive, neurological, and psychiatric symptoms of post-COVID-19 syndrome. The team, led by Daniel Martins of King's College London, points to sustained activation of microglia and astrocytes, the brain's resident immune cells, alongside blood-brain barrier breakdown and persistent cytokine signaling. That combination keeps the central nervous system's immune response switched on long after acute infection ends, and the authors argue this sustained activity underlies lasting brain dysfunction.

Long-COVID's mystery gut symptoms just got a biopsy-confirmed anatomical culprit: researchers found the vagus nerve's cholinergic wiring in the stomach lining is measurably thinner in Long-COVID patients, not just misfiring.
Italian researchers ran a case-control study comparing 12 Long-COVID patients against 8 controls, all undergoing routine gastroscopy. They biopsied stomach tissue from the fundus and antrum and stained it with two markers, PGP 9.5 to catch all nerve fibers and VIP to isolate the cholinergic ones tied to vagal signaling. This let them directly count nerve fiber density in living tissue rather than infer damage from symptoms alone.

Long COVID researchers have found that blood vessel cells are dying a specific, violent death, and it tracks with exactly how sick patients feel.
Researchers at the University of Miami Miller School of Medicine and the Miami VA Healthcare System ran a cross-sectional study comparing 73 adults with Long COVID against 41 age- and sex-matched people who recovered from COVID-19 without lingering symptoms. Everyone got standard Long COVID assessments, including symptom scales, autonomic function tests, heart rate variability, and vascular reactivity measures. Researchers then ran spectral flow cytometry on blood samples to check immune cells and circulating endothelial cells for markers of cell stress and cell death.
Long COVID patients scored worse on symptom severity, showed impaired heart rate variability, and had weaker blood vessel reactivity than controls. Their monocytes, neutrophils, and eosinophils showed elevated markers of necroptosis, a programmed inflammatory cell-death pathway, alongside signs of autophagy stress. Circulating endothelial cells, the lining cells that normally stay put inside blood vessel walls, showed up in far greater numbers in patients' blood and carried heavy activation of those same death and stress pathways.
Patients also showed increased clumping between monocytes and platelets, and between endothelial cells and platelets, a signature consistent with a blood that is primed to clot. Elevated necroptosis marker levels in the circulating endothelial cells correlated strongly with how severe a patient's symptoms were and how disrupted their autonomic nervous system had become. That link ties a specific molecular event directly to the fatigue, dysautonomia, and cardiovascular complaints that define the condition.
Why it matters: This gives Long COVID a measurable biological signature beyond self-reported symptoms, something patients fighting for diagnosis and disability recognition have needed for years. The endothelium, the tissue lining every blood vessel, appears to be under sustained attack long after the initial infection clears, and the resulting cell death and prothrombotic state may explain the persistent vascular and autonomic dysfunction so many patients report. The researchers point to necroptosis and autophagy pathways as potential drug targets, opening a route toward treatments aimed at the underlying cellular damage rather than just symptom management.
