Okay hear me out, i have a theory that Ragatha's mother caused her eye to be injured

in this scene where Caine is "putting them in their place" Ragatha is sitting in front of her mother looking as if she is going to be scolded like she did something wrong or knows what might happen and we can deduce that from how abusive her mother was in previous episodes

so, knives are thrown at her...

and as a result, a knife stabs her eye. Exactly in the spot where her button is. The thing is, the position of this button never changes so we can assume she also doesn't have her right eye in the real world. In which case was probably caused by her mother who may or may not have used a knife is the real world.

we also could assume this is her mom since it is a taller version of Ragatha. She doesn't know how her mother looks but knows she has one so she just sees her as an older but more fearful looking version of herself looking down on her.
