The MRI shows an L1 collapse but there are multiple bones involved giving a "moon crater" appearance with lamellated signal. The CT scan shows osteolytic lesions with sequestra explaining the MRI appearance. We did a biopsy. Not All Air Entry is Osteoporotic Collapse This 75-years old man came with backache and a compression # of the D12 vertebral body showing abnormal soft tissue. The PET/CT shows abnormal uptake and likely peri-osseous soft tissue, the CT scan showing osteolysis. A CT guided biopsy was performed. Pus was aspirated and a focus of air is seen within the aspirated cavity. This air however, unlike the last case, does not fill up the vertebral body or extend into other areas as with benign osteoporotic collapse, but remains localised in the void created by the aspirated material. #tuberculosis #tuberculosisspine #osteoporoticfracture #ctguidedbiopsy (at Picture This by Jankharia)
Air Entry and a Confirmed Diagnosis This is an 85-years old lady with a compression # of the D12 vertebral body with posterior retropulsion and conus compression. It looks like a benign osteoporotic compression fracture, the T2W image showing central cleft with fluid, with no osteolysis or soft tissue. The lady was not improving and the MRI report raised a suspicion of metastasis / tuberculosis that the patient's relatives were concerned about. A CT guided biopsy was therefore asked for. A costo-vertebral approach was used from the left. As soon as the first biopsy was obtained, air gushed into the cleft as seen on the axial image. This typical happens only with benign compression fractures. The sagittal image obtained after the procedure shows this well. The CT scan also rules out osteolysis and abnormal soft tissue. 3D Models - Understanding Complex Acetabular Fractures This is a 55-years old man with a complex right acetabular fracture predominantly involving the posterior wall and roof with comminuted fragments. He has been operated in the past. To plan surgery now, it is necessary to understand the current status in detail. While 3D images help, there is nothing to beat a 3D model that can be held in one's hand and rotated. The surgery itself can also be planned on the model. When MRI Scores Over Plain X-rays This 65-years old lady presents with pain in the left shoulder. A radiograph shows an osteolytic lesion with an aggressive appearance. There is mild expansion and an altered trabecular pattern. There is no characteristic diagnosis at this stage and the differentials include metastasis, myeloma, lymphoma and perhaps a primary bone tumor. The MRI though clearly shows a cartilage tumor, which given the length of the lesion and the endosteal scalloping is suggestive of chondrosarcoma. A Benign Googly This 20-years old lady presented with pain the knee. Her radiograph shows a benign looking lesion with sclerotic rim involving the lateral cortex of the distal femoral metaphysis. The MRI shows a lesion with lobulated hyperintensity and marrow edema while the CT shows the sclerotic rim well. The possibilities included a non-ossifying fibroma with a possible microfracture and a possible chondromyxoid fibroma. A CT guided biopsy was performed. Lighting Up This 70 years old man had a total knee replacement 5 years ago. He came with fever. A PET/CT was done, which shows intense uptake around the knee joint with effusion and synovial thickening. Touch Me Not This 50-years old patient came with shoulder pain following vigorous overheads in the gym. The radiograph was normal. The MRI shows a small cartilage lesion in the right humeral neck. This is an incidental benign cartilage lesion that is not significant, not the cause of the patient's pain and should be left alone. Green is Gout Dual Energy CT (DECT) and Gout This 58-years old man presented with a painful swelling over the radial aspect of the wrist. The radiograph showed an osteolytic lesion. Because of his pacemaker, a CT scan was done, which showed multiple osteolytic lesions in the distal radius and carpal bones with calcified soft tissue. A biopsy was done, which showed some form of crystal arthropathy. His serum uric acid levels were elevated. A dual energy CT of both elbows, wrists, hand and both knees, ankles and feet was performed. All the blobs of "green" reflect the presence of urate crystals, typical of gout. Usually in the Indian setting, florid tophaceous gout like this is not common. But in this patient, it has allowed us to clinch the diagnosis. #ctscan #dect #gout #dualenergyct (at Mumbai, India)
The Unfolded Infected Rib Rib Tuberculosis and CT Scan This is a 34-years old man with a swelling over the left anterior chest wall. The CT scan shows the collection as well as the osteolytic lesions of the ribs and the sclerosis, typical of an infection like tuberculosis. The unfolded rib image however puts it all in perspective. The Exponential Power of Complementary Studies This is a 59-years old with a one-year old left THR with one episode of infection in the past for which he was re-explored. While the MARS MRI shows a collection along the lateral cortex with marrow and muscle edema, the MARS CT scan clearly shows the osteolytic lesion along the lateral cortex at the site of the collection. This is peri-prosthetic infection. Granulomatous Cardiomyopathy This is a 48-years old with supraventricular tachycardia. The cardiac MRI shows mild to moderate hypokinesia of the basal anterior wall and antero-septal segment with mid myocardial enhancement suggestive of fibrosis with necrotic small mediastinal nodes. Granulomatous cardiomyopathy (GM) occurs due to sarcoidosis or tuberculosis.
