I'm just now realizing that Carmine probably ment like therapy patient, but this line had me thinking for like a day of Mad Scientist Carmine. Like imagine that Carmine got infected with his dimension's version of the Ruin virus and it just made him go insane and start "experimenting" on people. Said experimentation just being torturing them to death basically.
