For this concept, the only difference between Francos' family situation would be him being born as a girl. Given the time period (and the fact that Franco seems to be an only child from what is currently known), the very fact Francesca was born female would be an immediate source of her father's disappointment adding onto that having birth defects only pushing her father towards almost resenting her for simply being born. As she grew up, she would not feel able to act/dress feminine due to not having many female role models growing up in the mafia (other than her stepmoms) and feeling like the only way she would earn Salvatore's approval was by acting more like a boy and doing everything she could to help with her fathers business. I think that one of the only forms of affection she would ever end up getting from her father as a child is the nickname Frankie. I think much of her backstory with the mafia would be the same as Franco's up till around 14 years old. Based on the fact that Salvatore had multiple different wives, I kinda would assume that many/all of them would be gold diggers looking for men with money and power. I believe that as she got older and began to look more to her female role models, she would likely become more and more promiscuous and kinda bratty. I feel like she would develop almost impossibly high standards, wanting to be treated like a queen both in and outside the bedroom, expecting men to bend over backwards to make her happy and if they didn't meet her standards fast enough she would just kill them. Instead of being thrown out the way Franco was I think that she would get thrown out after she managed to seduce someone important within the mafia and killed him when he didn't please her in one way or another. Salvatore would then consider her a liability he no longer was willing to deal with and force her to leave. After that, much of her backstory would be the same as Franco's, the only real difference being that she would seek out men who were willing to be her sugar daddy (i think in part it would be men who pitty her based on her birth defect's effects on her appearance) and once they would get even slightly irritated with her bratty behavior she would kill and mutilate their bodies (She would totally cut their dicks off and stick them back in their mouths).
