The major chord that cures nightmares
Oppressive, frightening, nerve-wracking: nightmares are particularly disturbing dreams. They are considered pathological when they occur frequently (>1 episode per week) and cause daytime fatigue, mood alteration and anxiety. Although Imagery Rehearsal Therapy (IRT) has shown some effectiveness, some patients do not respond to this treatment. A team from the UNIGE and the HUG has developed a promising new technique combining this classic therapy with the Targeted Memory Reactivation (TMR) method. Thanks to this new therapy, the patients’nightmares decreased significantly and their positive dreams increased. These results can be found in the journal Current Biology.
Nightmares are dreams with strong negative emotions that occur during rapid eye movement (REM) sleep. Clinicians distinguish them from simple ‘‘bad dreams’’. In contrast to nightmares, the latter seem to have a useful function in promoting the regulation of emotions. Scientists also make a distinction between traumatic nightmares - i.e. linked to a state of post-traumatic stress - and nightmares without traumatic origin.
According to the ‘‘International Classification of Sleep Disorders’’, established by the American Academy of Sleep Medicine, nightmares become pathological when they are recurrent and have an impact during the day causing, for example, fatigue, anxiety, dysphoria or intrusive nightmare imagery. This is known as ‘‘nightmare disorder’’ and is an increasingly common reason for medical consultation.
Treating nightmares
Imagery Rehearsal Therapy (IRT) is frequently used to treat this disorder. This cognitive technique requires that the patients imagine alternative and positive outcomes to their nightmare scenarios every day for five to ten minutes. ‘‘After two weeks of practice, it has been shown that the frequency of nightmares decreases,’’ says Lampros Perogamvros, a privat-docent in the Department of Basic Neurosciences at the UNIGE Faculty of Medicine and a senior clinical fellow at the HUG’s Center for Sleep Medicine.
However, some patients are not receptive to this method. To overcome this limitation and boost the treatment process, Dr Lampros Perogamvros and his colleagues have coupled IRT with the Targeted Memory Reactivation (TMR) method. By sending specific stimuli to the brain of the sleeping person - often odours or sounds previously associated with recent experiences - it is possible to reinforce the memory of this experience. In this case, the aim was to reactivate memories related to the IRT exercises.
A piano chord played every ten seconds
The team from the UNIGE and the HUG gathered 36 patients suffering from the non-traumatic type of nightmares. Two groups were formed: one to practice the coupled therapy, the other the classical therapy with IRT only. ‘‘We asked the patients to imagine positive alternative scenarios to their nightmares. However, one of the two groups of patients did this exercise while a sound - a major piano chord - was played every ten seconds. The aim was for this sound to be associated with the imagined positive scenario. In this way, when the sound was then played again but now during sleep, it was more likely to reactivate a positive memory in dreams,’’ explains Sophie Schwartz, a full professor in the Department of Basic Neurosciences at the UNIGE Faculty of Medicine and the Swiss Center for Affective Sciences.
Each participant was then given a sleep headband containing electrodes that measure brain activity. At home, thanks to this device detecting the different stages of sleep, the piano chord was replayed every ten seconds each time the patient reached REM sleep. The exercise was repeated every night for two weeks.
More efficient and more lasting impact
At the end of the experiment, the frequency of nightmares decreased in both groups, but significantly more in the group where the positive scenario was associated with the sound. ‘‘Moreover, this association resulted in an increase in positive dreams,’’ says Alice Clerget, a master’s student in the Department of Basic Neuroscience at the Faculty of Medicine, who actively participated in the study. Finally, the benefits of the coupled treatment were still perceptible three months after the experiment, with patients in the TMR group still having fewer nightmares than those in the group without TMR.
‘‘While the results of the therapy coupling will need to be replicated before this method can be widely applied, there is every indication that it is a particularly effective new treatment for the nightmare disorder. The next step for us will be to test this method on nightmares linked to post-traumatic stress,’’ concludes Lampros Perogamvros. These results also open up new perspectives for the treatment of other disorders such as insomnia and other symptoms of post-traumatic stress, such as flashbacks and anxiety.

I was in high school and heard I was suspended for wearing goth clothes (not a familiar place I just remember it was dark with tall solid bluish gray walls). In high heel boots, I think a necklace and turtleneck and hat like the guy in Tokyo Babylon (out of dream I have been playing a dress up doll game like this), walking past crows of people to the exit. For some reason I started running, and I realized how well I was doing at running in these shoes and that of course this is uncool to the people watching.
It was raining outside, my mom standing there in the parking lot waved at me (not a familiar parking lot). A bunch of rodent-themed art was in parking lot too. One I remember was a white trash bag stuffed/bound into that white rat shape with stuff glued on to a convincing likeness at a glance. I think I also remember a scary angry painting on big piece of cardboard tied to a fence. In the car mom tells me she was bitten by a rat and it cut off her connection to our blood line and that's why she cant feel love from any of us at home.

In my dream a debt collector came to my door and took my money that was in a basket, I ran after him and ended up in his car, while he is driving he abandons the car. I cannot find the breaks and the navigation fooked so I’m driving through ppl’s homes. A coppie comes and I manage not to hit anyone and park in the lot of a building painted by children. The cops name is inuyasha and doesn’t care. They slip a zine into my jacket with their drawings and says they loved me before he even met me.lmao

When I was a little girl I had a recurring dream that still really freaks me out. In the dream my younger brother and I were standing on a dock by a bay and it was nighttime. I remember everything had a vaguely reddish filter over it. Anyways, my brother ends up falling off the dock into the murky water below, and when I somehow fish him out he's become shriveled up, kinda like a shrunken head. I had this dream at least twice if not more when I was around seven years old.
I dream about slime a lot people who know me know this isn't different and comes up when I know someone is super shady, which is almost consistently was heavy around the time I know my ex bestfriend was jealous and plotting on me. I dreamt about a cave and me dodging pools of slime smelling damp mud and wanting to immerse myself in the mud and bury myself I wanted to feel cold I wanted to feel secure I was pushing everyone away who wanted to come with me in the cave. I pushed away the slime trying to come over my mud/clay cocoon I was creating I sank deeper into this soft spot of clay/mud my bare feet found. There were glowing rocks here and there and nobugs mud was in my hair and I was breathing through perforated holes until i lost consciousness in my dream. I haven't dreamt in 4 weeks
i had a dream one of my roommates was replaced by someone else, and our whole apartment was subtly different. and this new roommate was trying to hatch chicks by boiling eggs, and i like knew that wasn't right so i yelled at them to stop and put the eggs back. cut to cardboard egg carton, a chick sitting in each cup, which i thought was pretty smart. then we were trying to feed the chicks this beige goop, but they were really still, not like, snapping it up like they should. we were just putting it in their mouths with tweezers and hoping it would go down. then one of the chicks didn't have a head, just this dark exposed neck hole, and i tried putting a piece of food in there before realizing it was a lost cause. and then all of a sudden all the chicks were kind of melting apart as we fed them, like they were made of wet tissue paper, and i knew the roommate hadn't boiled them for long enough. so i threw the chicks out and felt terrible about it.
worm dream
i was an old woman in this dream, and i was in a garden. it was at dusk and i remember it being somewhat overcast. i remember trying to harvest some sort of root (it may have been parsnips) and every time i would try to pull one of these things out of the ground it would just be a giant grub. later i really needed to get to the bathroom but apparently the toilet was in a cave. when entering the cave it was pitch black but i could tell there were other people in the cave, staring at me. when i finally found the toilet (a hole in the ground) i realized the walls of the cave were covered entirely in worms and whip-scorpions. i tried fleeing from the cave but i couldn’t move due to all the worms and whip-scorpions. the dream ended and i woke up needing to piss.
i dreamed me, my mom, dad, grandad & two of my aunts were eating at a dinner table, the air and light were deep brown. bfore dinner i was upstairs with my mom, she’s getting ready in front of a wooden vanity mirror, i tell her all my teeth feel sharp. we go down … and then we get to talking about teeth again, she says ‘you weren’t joking? they’re all sharp?’ i say yes.. she tells everyone yeah we took her to the dentist and we’d get her teeth pulled all the time, crowded and sharp. but even after that, there’s this third volley of teeth coming up, she spits some out every time she eats .. some people here are weredogs and just now i’m realizing im a wereshark. my mom asks, 'are you going to be ready?’ i laugh & say yeah i’ll just grow some gills. but really she’s asking if i’m gonna be ready to feed. my dad says i dont know how to hunt yet. i imagine myself being dropped from above into this dark blue bay, & i don’t know if i should go the first time full or hungry.. probably full. then my grandad’s hand trembles as he pours something, and my dad looks and says 'you’ve got a little cardiology on your left side.’
