What I say: So the decay has reached the nerve of your tooth so there is 3 options at this points. 1- do nothing 2- Do a root canal treatment that will take about 4 appointments where we take the nerve and BLAH BLAH BLAH or 3- I can just take the tooth out today.

What I am thinking: The hole in your tooth is huge and gross and you already have about 10 teeth missing and you never show for your appointments so lets pull this sucker out and be done with it.

What I say "Just having a quick look around there is a bit of plaque build up in some areas. We might have another chat about how to get to those places when brushing today"

What I am thinking "I could smell the plaque on your breath before I even looked into your mouth. Obviously you've forgotten how to brush you teeth since last time I showed you so I will show you again today in the hope that your tooth brush reaches your back teeth in the near future."
