They are paid hourly, and most not enough. To imply the entirety of industry and the people within is corrupt with no one coming forward about it is comical. And again. It's true that I don't know about other countries and that I hastily assumed the United States. That is my bad. But there is no need to manufacture illness to do so- it's insurance companies and hospitals that make most medicines and procedures insanely overpriced. It's hidden hospital fees and loans and payment plans you can get directly from the hospital... It's a very very flawed system that prioritizes money and it's killed many.
Hrt is actually one of the few medications that have escaped this, with the medicine being a reasonable price. I also have fibromyalgia, and my medication for that is literally 10 times more expensive for no reason and with no added complexity it's production. And medications absolutely required for life like insulin are infamously overpriced. Luckily gender affirming care is SOMEWHAT affordable compared to other conditions, but it's provided for free in many other countries. (For example an MRI, a common and important test, its at LEAST $10,000 for a singular one, and you usually require multiple)
The cost issues with healthcare are at a higher level then doctors and surgeons- they are paid reasonably, and if costs were to actually pay for the labor and facilities then it wouldn't be that expensive. It's a complex issue mostly tied up with insurance companies that raise the prices at a corporate level, and most the money actually goes to them and higher executives, leaving many hospitals underfunded actually.
As for the positive effects of hrt- aside from studies, my own mental and physical health improved tenfold, and I'm very involved in the trans community in my area. And while it's not the solution for all trans people, I've only seen it improve lives personally. Plus it made me super hot.
