💪🏼Arm Lift💪🏼 Arm Lift - also called “Brachioplasty” is a procedure where excess skin of our arms being removed surgically. It can be done as stand alone, or supplemented by liposuction for extra toning. . . This young patient of mine had underwent one of the earliest arm lift I did. She had a chunk of skin with fatty tissue excess hanging over her both arms, after a weight loss. This flabby part of one’s arm is also described as “bat wing arms”. . . A batwing arms can be toned in certain limit with toning exercise. But, in some people, no matter how much they exercise, these just won’t go. Thats where surgical removal comes to play. It is imperative to know that in a brachioplasty, there will be scar, but the incision is made such that the scar will be hidden at the inner side of you arms. . . I like to supplement any extremity body contouring skin removal with liposuction. Although its optional, it adds better contour and lower the post operative complications. Cleft Palate repair is a bread and butter in plastic surgery. But this palate repair was a little bit harder as previously the kid underwent both cleft lip repair and anterior palate repair simultaneously. (Commonly in my practice, the first stage is only lip repair without anterior palate repair.) Therefore, the anterior palate was more scarred, and I had a more difficult dissection during this 2nd stage cleft palate repair. . . The extra task is worth my effort though, as the early outcome studies of doing both lip and anterior palate repair simultaneously in the 1st stage has its positive effects.(See reference below). Im definitely going to learn how to do that for my future lip repair. . . Immediately post operative, the midline cleft is closed, but there will be 2 raw defects on the periphery. Both of these raw defect is normal and expected, and they will close spontaneously after 2 weeks. Swipe left to see the outcome after 1 month. At this time, the palate is fully close, as well the 2 defects on the side. There are still remaining absorbable sutures in the front that will disintegrate on its own later on. Alhamdulillah. . . Lower limb reconstruction prioritize function before form. It was a privilege to be able to cover this patient’s wound, that was exposing quite a large part of his tibial bone after a tumor wide excision located on his shin. . . Skin grafting is not adequate to cover this type of wound. So, I created a local flap (a soft tissue composing of skin, fat and fascia) adjacent to his wound and transposed it, enough to cover all of his exposed bone. The extra wound that I created was then covered with a skin graft (a thin sheet of skin taken from his thigh), creating a third superficial wound that heals spontaneously later on. . . Tumor out, flap in! 😁 . . Its ironic how we need to borrow part of our bodies one after another to substitute other areas of inadequacy at the same time, and end up with complete closure of all wounds. . . Gillies’ 10 Commandments No. 6: “Thou shalt treat thy primary defect before worrying about the secondary one.” . Happy World Plastic Day July 15th! Did you know that July 15th is World Plastic Surgery Day? . . I was actually trying to microblog a simple wish to everyone yesterday (July 15th), when my medical officer suddenly called me and informed me about a bleeding anastomosis of one of my patients. Jumped out from the bed, and straight away changed my clothes, breezed through the city final rush hour traffic jam and the next thing I know, I was already in the operative theatre for exploration . . While on the way to the hospital, I had an instant crash course of what to do with my sifu. With the ongoing adrenaline rush (while pretending to be calm), I tried to absorb everything I can, at the same time questioning myself if I can get through it. . . Anyway, back in the operative theatre, I explored the area, and after I found the bleeding source (ruptured anastomosis by the way at superficial femoral artery), with my left index finger stopping the hemostasis, my right hand started to dissect away all the blood clots, slough and fibrous tissue surrounding the area (majorly infected wound bed made everything a jungle down there!). Everything is very friable and there are blind spots everywhere (i.e. risk of injuring the vessel wall because of obstructed view, that may cause massive bleeding). . . Right after I completed my vessel dissection, managed to secure distal and proximal vessel control, and free the area from the ‘inflammatory jungle’, my sifu Prof Arman arrived(Thank god!). The superficial femoral artery was badly damaged due to the infection and needed to be resected. We then proceeded with a bypass with a saphenous vein graft was done for this patient to save the lower limb. . . Microsurgery is still a long road for me, every challenge faced, is another step of milestones. Count your blessing!. . . Happy World Plastic Day. Chest wall reconstruction. . . Chest wall - the part of our trunk that protects many important organs, including heart, lungs, and large vessels. An open wound of this area of our body need to be closed not only with adequate skin, but also obliteration of this area with durable soft tissue to protect the organs underneath. . . In this patient, with metastatic follicular thyroid cancer from the manubrium, extending to the part of both clavicle, 1st and 2nd rib and also some part of pectoralis major muscle, a large robust soft tissue is needed to cover the open wound after the tumor resection. . . Cardiothoracic team did the resection and my team and I took over for the reconstruction. Fortunately, the skin on resected on the other hand, is not so much, allowing us to use bilateral pectoralis major muscle rotation advancement flap to obliterate this area. . . Unlike few bilateral pectoralis major muscle advancement flap that I did before, which is simpler as compared to this case, this time lateral and lower attachment of the pectoralis major had to be released to allow both muscles to be rotated and advanced upwards, hence obliterating the mediastinal area. Plus, the skin above also was separated to free up the muscle further. . . In this case initially, we had 4 reconstructive plan, depending on intraoperative findings. Lifeboats, are very important in plastic surgery, as commanded by Gilles - Father of Modern Plastic Surgery in his 10th Commandment of Principles Of Plastic Surgery Number 7 : . “Thou shalt provide thyself with a lifeboat”. . . Scalp Reconstruction In Exposed Infected Titanium Mesh . . 2 weeks ago, I had a referral for a soft tissue coverage of an infected exposed titanium mesh. The mesh has been there for almost 10 years, but unfortunately it got infected and exposed within 2 weeks before she presented to us. . . Together with neurosurgery team, the infected titanium mesh is removed, part of the superficial part of the dura that is non viable is also debrided, leaving a partially calcified dura underneath. Despite after titanium mesh removal, the wound is still large to be closed primarily. Thats when plastic surgery input is needed in such cases. . . After an intra operative assessment, I decided to extend my incisions using the same scar, and advanced the forehead skin (free forehead lift 😉, rotate the contralateral scalp, and also rotate the ipsilateral scalp. Thankfully, all 3 advancement and rotations managed to close the exposed dura, and donor area still can be closed primarily. . . Because the wound closure has a little bit of tension in its approximation, a splinting negative pressure wound therapy was applied on the wound to facilitate the healing and the approximation. . . It was not a straightforward case for me, as the patient’s previous incision for her initial craniotomy surgery severed blood supplies from right superficial temporal artery and also supplies from frontal (supratrocheal/supraorbital arteries). Therefore, the planning for a local flap is limited because I could not burn any further bridges for blood supply, that I may need for future tissue expansion insertion when a new titanium mesh will be inserted. My lifeboat is a split thickness skin graft (simplest but it is certainly not the most ideal type of soft tissue coverage for cranial protection, and it can be quite a hassle to elevate a grafted skin from the dura underneath in future procedure).
