A Bigger View of Hair Loss
According to Dr.U’s publication, hair loss is more complicated than most of us realize. Beyond just DHT and low scalp blood circulation, it involves the effect of regulatory proteins on the phases of our hair cycle, as well as other factors such as oxidative stress and inflammation. Instead of one or two straightforward pathways, we have to now think of hair loss as a “cascade” of interrelated events.
Five Patient Cases

Eyebrow transplants are more common than most may think. Many people, women especially, desire the naturally curved eyebrows, that are neither too thin, nor too thick. However, this can be an unattainable goal for women of color.
Hair comes in a variety of types, from different colors to different textures. Women of color tend to have thicker and curlier hair follicles. If you’re a fan of the sleek bald look, you’re probably familiar with the quintessential culture and aesthetic of barber shops, like the one below.
There is often a tightly knit community within barbershop circles. These circles are generally inclusive of ethnic communities, such as individuals of Hispanic or Afro-Caribbean descent.
A less positive association of barbershops and the culture around it, however, is the prevalence of “Barber’s rash”. Barber’s rash is the outbreak of acne and itchy bumps after a close shave, usually at a parlor. Many people associate barber shop clippers with this rash. And men, especially men of color, can trace an outbreak on their back hairline to a visit at their barber’s.
However, these breakouts can be early signs of a more serious skin condition known as acne keloidalis nuchae - or AKN.
What Causes Acne on the Back of the Head?
While the exact cause of this skin condition is unknown, many medical experts have noticed a prevalence of its occurrence in young African American and Hispanic men. The small bumps, typically just above the neck, can easily be mistaken for a rash or razor burn. However, unlike those temporary irregularities, AKN does not typically go away on its own--often, it tends to progress into more severe stages.
The association of shaved heads and acne on the back of the head has spawned the hypothesis that the improper regrowth of hair after a shave causes AKN. Experts tend to agree, however, that the general onset is as follows:
The body mistakes its own hair as an invading pathogen and responds accordingly.
The follicle becomes inflamed, forming the bumps.
As the body attempts to heal itself, thick skin develops over the bumps.
Sometimes, tufts of hair can be seen sticking out of some keloid-like bumps on the back of the neck.
Why Is No One Talking About This?
The silence around this condition that affects predominately Black and Hispanic men is troubling. When left untreated, the bumps on the head can fuse together into a larger mass.
Part of the issue is simply a lack of knowledge on the condition, both on the part of the community that suffers from it, as well as the medical community in diagnosing it. Often, it takes a specialized physician, like Dr. Umar in Los Angeles, to identify and properly remove these AKN bumps.
There are treatments available for chronic acne on the back of the head. This Chicago native describes his experience as “freeing” since having had his AKN lumps removed.
Last year saw several interesting male grooming trends, gearing men toward longer hairstyles. However, the results of a poll we recently did on Twitter revealed that many people still prefer undercut and/or fade hairstyles.
So What Does This Mean For Male Fashion?
Longer hairstyles for men have often been associated with rebelliousness and an individualistic spirit. However, the growing acceptance of long male hairstyles such as the Bro Flow and the man bun have may have contributed to the rush of seeking styles that continue to push boundaries.
The Undercut V. Fade Debacle
The undercut hairstyle can accentuate the clean, straight lines of a shave at a barber shop by separating the hair on top from the back and sides. A fade shaves down the back and sides in order to create the appearance of fading into the scalp, hence the name. The undercut and fade are often used interchangeably, although they can be styled separately. The model in the photo above, for example, sports a man bun with an undercut, also referred to as a top-knot. It is not unheard of, however, for an undercut to have a fade. Often, this can help further accentuate the modern look of a fresh cut.
The Struggle for Men With Strip Scars
Although historically men have favored shorter lengths on the back and sides, the more recent phenomenon of scars due to strip surgery has left many men in an unforgettable position. Some men who are unable to wear their preferred hairstyle without drawing unwanted attention simply grow their hair out to shaggier lengths, much to their obvious displeasure. The star’s eye bags and pigmentation served him well in his role as an insomniac angsty teen--apparently so well that the special effects department took over where the makeup department left off and attempted to soften the prominence of bags.
Phoebe Tonkin, like most people with hereditary dark circles and under eye bags, has been on the receiving end of many questions along the lines of “Have you been getting enough sleep?” and “Are you alright?”, so much so that she felt the need to take to her Twitter and set the record straight. While useful in her role as an insomniac werewolf in the hit CW show The Vampire Diaries and its spin-off, The Originals, her dark circles garnered her unwanted concern off screen.
A running theme apparent in the roles of these respective celebrities is insomnia; their prominent under eye bags and dark circles help authenticate the exhaustion and irritability of their respective characters. It is doubtful, however, that these actors wish to project these qualities off-camera, which is why they are often covered up for public appearances.
