For me, it is canonical that Debra died in love with Dexter. She knew that Dexter was the only man she truly loved, and knowing that he would never feel the same way hurt her so much.

So, Debra told Quinn that she still had feelings for him — taking advantage of his feelings for her and trying to convince herself that she could go back to feeling the way she felt long ago, when they were both in a relationship — because she was afraid of ending up alone.

Dexter would leave Miami and a part of her believed she wouldn't see him again, and that hurt her. So when she realized she couldn't have the only man she truly loved, she gave the man who loved her a chance at a fresh start — even if she didn't love him back in the same way.

And she died in love with her adopted brother — without even having the chance to hear him say "I love you" first (it was always her who said it first), and I'm sure she would have been touched by that. She just wanted to be loved, and that hurts. A part of me likes to think that Dexter realized he loved her the same way when she fell into a coma and died, and that hurts even more. She would not have his reciprocity if she lived, but she did when she died; That's enough to kill anyone.

They were in love with each other their whole lives and it took Debra nearly her whole life to realize what it was. Then it took Dexter Debs life to realize she was everything to him. Nothing more tragic.
