Iâve started explaining to my patients that the 5.07/10g monofilament used in the USA to quantify diabetic peripheral neuropathy was originally set as the standard for people in India who had leprosy. The concept of a âprotective sensationâ threshold was if these patients could tell if their toes were being eaten by rats while they slept, and the 5.07/10g monofilament predicted this successfully.
This is why 2 point discrimination is so much more sensitive than monofilament testing for patients who have symptoms of neuropathy but otherwise have an intact monofilament exam - the standard was never intended to be used for other causes of neuropathy.
