Foolproof plan for sure
I know there's a popular headcanon that Grace's crew died because of feeding tubes malfunction (based on the paperwork Grace was doing right before the explosion), but — in the book he specifically says that even after the accident he kept dealing with that same paperwork on minor Hail Mary issues, so I doubt that feeding tube problem was left unaddressed. May I offer instead:
Grace was put into coma by the people who cared about him. They (especially Yáo being Yáo) probably double-triple-quadruple checked everything. They watched him sleep for those first few days — I doubt they went into coma immediately after leaving Earth's orbit. They probably talked to him, assuring him that he'll be okay.
Ilyukhina's coma procedure was probably supervised by Yáo. He made sure that everything was in order, but — he is just one man and he is not a doctor. There was much more room for mistakes.
When Yáo went to sleep, he was alone. He had to rely on the technology completely.
We know that he died first.

I love love LOVE a platonic relationship between Rocky and Grace where neither of them are normal about each other.
They’re best friends but by both Earth and Eridian standards everyone is like “Oh wow, they’re… close-”
Like little to no cares about privacy around each other because, well, fuck you aren’t getting much of that on a spaceship for multiple years now are you?
Wherever one goes there’s a 50% chance the other will be there too.
Whenever Grace talks to another Eridian he uses some sort of portable instrument to communicate but with Rocky they just speak in two separate languages at each other. It’s like seeing someone speak to someone in only Chinese and the other person only responds back in English.
Would be funny if Eridians aren’t very touchy but Rocky is basically clingy as fuck when it comes to Grace. Always tugging on his pants like a toddler to get his attention and stuff.
Grace just using Rocky in either his ball, enclosure or his xenonite suit as a backrest while he’s grading or doing work or something, Rocky fully absorbed in his own thing. Sitting as close together as physically possible while having zero interaction with each other. Parallel play at it’s best.
These two come in a pair. Do not separate.
All of this and more, fully platonically, people wonder if they’re the world’s first weird inter solar system couple and if they’re in some weird polycule with Adrian but no, they’re just like that.
It’s just Rocky and the human he adopted as a permanent part of his family that he will spend every possible second of the rest of his short life with because he won’t be here forever and he’s not going to waste any of his time for the world.
Also Adrian still loves Rocky and isn’t quite as close to Grace as he is but is also very fond of the weird awkward alien their mate brought home because Grace is Rocky’s family and that makes Grace Adrian’s family too.
grace and rocky get to erid and both of them are hitting every eridian squarely in the uncanny valley because they've both absorbed so much of each other that they feel both too eridian and not eridian enogh. rocky faces in the direction he's paying attention to and it feels like he's trying to hide something, the way he and only he of the eridians seems to always be showing the same face of his carapace when he speaks to somebody. he speaks in metaphors that only make sense to somebody who understands light-based vision. his intonations are funny, rising on questions when they otherwise wouldn't, falling flat, his sarcasm goes to space dry and restrained and comes back with such force it almost feels like he's lying instead of joking sometimes.
and grace who is so alien, but reflects eridian mannerisms that they've never considered strange before but coming from a creature so soft and strange, it just doesn't feel right. grace who stomps his foot twice when he asks a question, and it rings out flat and dull instead of the strong, sharp sound of an eridian hand on the ground. grace whose voice is so alien, so one-note, so full of articulations that are sharp and round and everywhere in between, but who echoes the melodies of eridian poetry and music, sometimes on accident but sometimes perfectly in time with what rocky translates him as saying, and later, perfectly in time with his keyboard.
rocky and grace who mirror each other more similarly than they mirror their own cultures. who speak to each other in a strange pidgin eridian that nobody else quite understands fluently. who share jokes from movie and tv shows from earth filtered through eridian folktales, plays, and musicals until they only make sense to the two people who know both cultures. they're not strange to each other, but it puts other eridians on edge. even adrian, at first. they've changed each other, you see.
