đš PART ONE : CIPA (Congenitive Insensivity to Pain with Anhydrosis)
âą COMMON SYMPTOMS
CIPA can be described as the insensitivity to pain and to temperatures, even extreme. During the pregnancy a mutation of the gene responsible for the reception of pain and temperature signals occurs to create permanent dysfunction. People with CIPA can also experience episodes of unexplained infections or fever, and have self-mutilating behavior.
CIPA is a condition present since birth and extremely dangerous for the patient since they are not aware of injuries. It means it is impossible for them to recognize the bodyâs alarm signals. Patients with CIPA lack any nociception and thermoception. However it is still possible for them to feel pressure, touch or pleasure since theyâre all unrelated senses.
CIPA causes significant developmental delays in children such as babies not knowing when to stop chewing, children or teenagers can harm themselves with unconscious habits like rubbing their eyes or biting nails. Similarly itâs not uncommon for patients with CIPA to neglect bowel needs and nutrition since they donât feel the pain their bodies are sending like an alert.
Anhydrosis is a serious condition all in itself and differentiates CIPA and CIP with how life-altering it is. Patients with this condition are unable to sweat properly, producing little to no sweat. Sweating plays a major part in temperature regulating, patients are prone to overheating, causing faintness and even death. Instead patients may experience dizziness, muscular cramps, general weakness and a strange sensation of âfeeling hotâ. However that latest symptom is not felt by people living with CIPA, endangering them even more.
âą TREATMENTS & SOLUTIONS
With these informations in mind we can easily say that Toby is a disabled character. CIPA is a heavily disabling condition that significantly impacts the patientâs life and that can be very much lethal. Patients with CIPA NEED special accommodations to have a seemingly normal life.
Accommodations may include :
â°âș Regularly checking the body for any injuries or odd elements. Keeping a close track on physical integrity may be life-saving in anticipating upcoming infections or diseases.
â°âș Wearing protective clothes or accessories to avoid self-harming like eyepatches, glovesâŠ
â°âș Having a danger-proof house for young children.
â°âș Teaching patients to recognize external signs of injuries or diseases (like teaching children that blood means danger), to pay close attention to their overall state and report any anomaly.
â°âș Regularly having medical appointments in case of hidden symptoms or undeclared pathologies.
The list above can apply to patients with CIP just like CIPA, but there are additional accommodations that can be put in place specifically for anhydrosis.
â°âș Wearing large and light-colored clothing, and covering the head with a cap or hat to protect from sunlight.
â°âș Staying in cool places, indoors or in the shade for example.
â°âș Taking cool baths or showers but not cold as it can cause thermic shock.
â°âș Staying hydrated and avoid consuming alcohol or caffeinated drinks.
â°âș Avoiding overexertion and intense physical effort.
â°âș Keeping a water bottle nearby at all times for an emergency cool off or drink.
â°âș Keeping a close eye on secondary sensations like dizziness, mouth dryness or sudden fatigue. To associate with precise observation of the environment and trusting loved ones.

âą HOW DOES TOBY LIVES WITH CIPA
Nota Bene : the following content is purely invented and stretched out of my research on CIPA. This is MY vision on how Toby lives with his disorder.
â°âș Toby definitely had a rough home life that wasnât eased at all by his disability. Since he grew up in a neglectful home, his CIPA would most likely be diagnosed late. He had been diagnosed at seven, before then his little accidents would have been brushed off as being the âreckless but silent kidâ. The troublesome one who never complains when they hurt. Because of this he often stayed indoors. On some days it was to protect him from hurt, on others it was for him not to cause trouble. His sister Lyra was the first to notice that something was very wrong with Toby, and ultimately was the one to cause his diagnosis.
â°âș Ironically the management of his condition got better once he became a proxy. Slenderman had all the interest to keep him alive, so he imposed a strict set of rules for Toby to follow. The constraint he was put through was only meant to last a few months, the time for Toby to get shaped to the manorâs life and learn how to live without risking getting killed every three days. The rules included not being left alone in dangerous environments (mainly the kitchen and armory), having mandatory meet-ups to learn about handling dangerous tools and seeing his mental and physical health getting examined.
â°âș Once he got used to his new life, the constraint lessened. Now, every week, Toby had to go to the infirmary to get examined by Nurse Ann for possible diseases and hidden injuries.
â°âș Toby carries a thermometer in his pocket, to gauge the temperature of the environments and surroundings elements (like drinks or the bath for example). It was offered by Nurse Ann so he could stop âget burned like an fucking idiotâ. He uses it now every time out of automatism, and it considerably lowered the risk of accidental burns.
â°âș Very early in his life in the manor, Toby was taken under Tim/Maskyâs wing. Naturally the proxy watches over him and taught him a lot of ways to be more careful with his health, but is also the one who checks on/treats his wounds the most. Since Masky/Tim is like a mentor, he was also the one to monitor Toby during training to avoid overexertion, mainly with timed efforts and careful heart rate measurements.
â°âș To avoid SH Toby wears protective gear, so his mouthmask and bandages or band-aids and gloves. He uses similar means for his anhidrosis, his clothes are baggy and/or loose to avoid overheating. When the hot season kicks in he protects himself from the sun with his hood and/or a cap.
âą HABITS OF HIS
â°âș With time Toby also learned to go to the infirmary when he was feeling overstimulated. The place is quiet and usually occupied only with Nurse Ann, sometimes Eyeless Jack. When the manor feels too much, it happens for him to hide in one of the beds. Nurse Ann just lets him as long as he doesnât mess the room. On her good days she brings a cup of water, but most of the time she ignores him.
â°âș When Toby enters a crisis, he follows a strict path to find a refuge. First he runs to the infirmary, and goes to hide in it the time his crisis lasts. If the infirmary is locked heâll find the first dark corner or even run outside to get away from all the noise and the static. He can stay in these temporary havens for hours without moving.
â°âș Unfortunately Toby falls into self-destructive behaviors under stress. Heâs prone to self-harm and will chew on his lips, scratching and biting his nail folds and cuticles or even the soft parts of his hands to the point of bleeding. Because of this his lips are scarred, and his hands constantly covered with more or less fresh wounds.
â°âș Once a day Toby does a full check up on his body, usually just before bed. He reports any anomaly to either Tim or Nurse Ann.
