medication reconciliation

please carry a current list of your medications, their doses, and the frequency with which you take them on you.

physicians are notoriously bad at reconciling medication lists (despite knowing that medications contribute to a good number of hospitalizations) because it takes so much time and effort to go through the entire medication list and try sort everything out.

the goal of physicians who are called down to the emergency department to admit a patient is to quickly see a patient and come up with a diagnosis and plan after listening to their story and doing a physical exam. efficiency is the key to deliver timely care to the patient as well as to the other patients that physician is responsible for.

spending sometimes hours tracking down a caretaker or faxing consent forms to closed medical offices or calling and calling and calling different emergency contacts at all odd hours of the night to try figure out what medications a patient is on is beyond frustrating because it causes a delay in care not knowing what medications to provide and which ones to avoid.

and unfortunately, no, physicians are not taught what shape, color, and size each medication looks like so for the most part, they are unable to recognize what pill you might try to describe.

so please, carry your medication list with you if not your medications themselves in their prescription bottles. this is important to the physician as well as for your care.
