I thought it is because Cater is eating sweets which he heavily dislikes. I mean I already knew right away that the birthday series involves sweets and he will likely hate it.

Cater does dislike sweets, but there are ways to dress them up to be more savory or less sugary. For example, dark chocolate or fillings like crispy bacon or chili powder.

I also don’t think he’s necessarily make a face because he’s unhappy or disgusted? I actually think he looks pretty neutral or nonchalant here; it’s the makeup and slanted down eye shape that makes Cater appear gloomy.

Even if Cater doesn’t like sweets, he still pretends that he does in public. Trey calls him out for this in book 1-28:

In the context of the La Bonbonnière card, Cater is dining in a pâtissière with at least 1-2 other people: Yuu and Vil (his duo partner). Yuu might be familiar with Cater’s secret if you want to follow the main story continuity, but Vil certainly isn’t. Cater would try to maintain his facade, even if vignettes like Cater’s Labwear suggest that Vil can see through his act or doesn’t always believe his claims.
