I can't believe we live in a world where there's an AI company unironically called "Palantir," and it isn't a parody. It's a real thing. I remember seeing a picture of an advertisement on here and thinking, "This HAS to be a joke. This is too on-the-nose to be real. They wouldn't honestly name an AI company Palantir, after the Seeing Stones from Lord of the Rings that are supposed to offer knowledge, but famously also might be feeding you misinformation from evil sources because 'we do not know who else may be watching.'" But then here I am listening to the BBC News discussing why the CEO of Palantir just published a Manifesto that sounds like it was written by a supervillain.

Haha my gf was so right about my sister not meaning it when she asked me to take the kids when their lease runs out so she can go to rehab and then shelter to get back on her feet, but was just trying to manipulate me into offering to either co-sign or let her live with me…

It’s a no for sooooooo many reasons lol. Simply not ever happening. Everyone that lives with her ends up in jail because she either makes false accusations or drives them insane! Plus, I will not become more complicit than I already am in the way shes raising those kids.

A few weeks ago a patient got intubated on the floor for respiratory depression and altered mental status and when I was helping the nurse admit him to icu I noticed he had 2 fentanyl patches on. This was 15 minutes before shift change. Nurse didn’t know if he was supposed to have two and was more focused on other stuff so I dug into the chart, found that two days prior a nurse on the floor couldn’t find the patch and so she placed a second one. I took photos, pulled both off, wrote a note, informed the doctor, informed two assistant nurse managers and filled out an incident report…. Patient was extubated and downgraded later that day once we stopped ODing him on double fentanyl patches. So why am I having the ANM asking if I informed the doctor three weeks later? Primary nurse said she’s gotten like 4 calls about this. We (mostly me) should literally be getting accolades for our good catch.

AND I had all of the charting up to date for that nurse by the end. I am literally Superman.

The stemi leading to mvc and cardiac arrest patient that I spent 5 hours helping with last week got extubated last night probably one of my best wins. His EF recovered to like 50%, his brain works and his kidneys still work. He had splenic and chest wall bleeding as well as impella site bleeding plus he was in mixed cardiogenic/hemorrhagic shock. I was the one who held 30 minutes of pressure at his bleeding impella site, helped get him to CT, made sure the nurses running the level 1 were doing balanced transfusion instead of slamming him with RBCs, made sure we were warming him, made sure we checked calcium and magnesium, made sure mag was given fast when he started having v tach runs again which I think prevented another arrest. It was all newer nurses than me so I really got to shine lmao. We were so prompt with everything that if you looked at his labs, vitals, and urine output you’d think nothing happened. By 7am I had that bleeding stopped and pt was in sinus rhythm. Most patients I don’t feel like I did much more than what anyone else would do but this one I definitely contributed. Recheck coags after were literally perfect after being fubar prior.
