While I agree that atypical dysphoria is not synonymous with BIID/BID, you are incorrect that feeling like you should have a chronic illness can't count as a BIID need. BIID doesn't have specific criteria related to which needs "count."
The following is a list of required criteria from the ICD-11, as the DSM-5 does not include BIID/BID.
1. An intense and persistent desire to become physically disabled in a significant way (e.g., a major limb amputation, paraplegia, blindness) accompanied by persistent discomfort or intense negative feelings about one’s current body configuration or functioning.
2. The desire to be disabled results in harmful consequences, manifested by either or both of the following:
2A. Attempts to actually become disabled through self-injury have resulted in the person putting their health or life in significant jeopardy.
2B. Preoccupation with the desire to be disabled results in significant impairment in personal, family, social, educational, occupational, or other important areas of functioning (e.g., avoidance of close relationships, interference with work productivity).
3. Onset of the persistent desire to be disabled occurs by early adolescence.
4. The disturbance is not better accounted for by another mental disorder (e.g., Schizophrenia or Other Primary Psychotic Disorder, in which, for example, a delusional conviction that the limb belongs to another person may be present, Factitious Disorder) or by Malingering.
5. The symptoms or behaviours are not better accounted for by Gender Incongruence, by a Disease of the Nervous System, or by another medical condition."
ICD-11 on Body Integrity Dysphoria
Nowhere does it say that there is a specific, exclusive set of disability needs that count as BIID/BID. There have been reports of people desiring deafness, muteness, multiple sclerosis, cerebral palsy, and various forms of incontinence. On a BIID/BID forum I frequent, I have also seen people having needs regarding ostomys, including tracheostomy tubes, and one person who desired a large portion of/their entire small intestine removed. I myself have multiple needs, one of which is a feeding tube.
Not everyone who claims to be "transabled" falls into the criteria of BIID/BID, but this is because the disorder must impact your life in a negative way and cause you harm. The majority of pwBIID/BID I've met have had serious issues with suicidal ideation and self-harm, which is why we call our desired disabilities "needs." We don't say we "want" to be an amputee or "want" cerebral palsy. We say we need it because for many of us, it's a matter of life and death and truly a need. The criteria does not exclude any specific needs. it just gives a small list of examples of what a person's needs may include. E.g. means “for example.” (It stands for exempli gratia in Latin.) It is used in much the same ways as 'for example,' coming before an item or list of items.
Deafness as a need is common amongst forums for people with BIID/BID, yet it is not listed in the small example list because they could not possibly list every single disability ever since that's not the point of the criteria. If there was a strict selection of needs, they would outline that in the required criteria section.
