A groundbreaking gene therapy treatment which boosts a patientâs own immune cells has been shown to clear disease from one third of terminal patients.
US pharmaceutical company Kite Pharma released results from the first six months of its trial of the new treatment, called CAR-T cell therapy.
Some 36 per cent of the 101 patients on the trial were still in complete remission at six months, and eight in 10 saw their cancer shrink by at least half during the study. âThe numbers are fantastic,â said Dr Fred Locke, a blood cancer expert at Moffitt Cancer Center in Tampa who co-led the study.
âThese are heavily treated patients who have no other options.â
The treatment, which has been dubbed âa living drugâ by doctors, works by filtering a patientâs blood to remove key immune system cells called T-cells, which are then genetically engineered in the lab to recognise cancer cells.
Cancer cells are very good a evading the immune system, but the new therapy essentially cuts the brakes, allowing immune cells to do their job properly.
Martin Ledwick, Cancer Research UKâs head cancer information nurse, said: âThese results are promising and suggest that one day CAR-T cells could become a treatment option for some patients with certain types of lymphoma.
âBut, we need to know more about the side effects of the treatment and long term benefits.â
Patients in the study had one of three types of non-Hodgkin lymphoma, a blood cancer which affects 13,600 patients in Britain, and had failed all other treatments. Most patients with such an advanced condition only live for six months but half of the trial group are still alive nine months since the trial began, and a third may be cured.
Dimas Padilla, 43, of Orlando, who was warned his case was worsening after chemotherapy stopped working, is now in complete remission after undergoing the therapy last August.
After learning his cancer was probably terminal he said: "I was thinking how am I going to tell this to my mother, my wife, my children,â he said.
After CAR-T therapy he saw his tumours âshrink like ice cubesâ and is now in complete remission.
âThey were able to save my life,â Mr Padilla added.
However there are still concerns that the treatment has significant side effects, and can even kill some patients, as it puts the immune system into a state of over-drive. During the trial two people died from the therapy, rather than their cancer.
Of the study participants, 13 per cent developed a dangerous condition where the immune system overreacts in fighting the cancer, and roughly a third of patients developed anaemia or other blood-count-related problems.
Nearly one third also reported neurological problems such as sleepiness, confusion, tremor or difficulty speaking, but these typically lasted just a few days.
The scans show how cancer has disappeared after just three months, and the remission has continued The scans show how cancer has disappeared after just three months, and the remission has continued Full results will be presented at the American Association for Cancer Research conference in April and the company plans to seek approval from European regulators later this year.
âItâs a safe treatment, certainly a lot safer than having progressive lymphoma,"said the cancer instituteâs Dr Steven Rosenberg,
Other companies, such as Juno Therapeutics, have had to halt trials into CAR-T treatments following patient deaths.
In the past few years, next-generation cancer drugs have started trickling into the clinics, including a smart inhibitor that block a specific mutant kinase (V600E-B-RAF) and antibodies that can induce T cell-mediated rejection of certain tumors (anti-CTL4 antibodies). Another promising approach is to genetically modify T cells to attack tumors and then infuse the cells into cancer patients. Indeed, this strategy is currently entering clinical trials, specifically with T cells engineered to express the chimeric antigen receptors (CARs).
Movie: Here T cells (gray) are engineered to express chimeric antigen receptors (CARs) to redirect T-cell specificity to target CD19-positive tumor cells, expressing EGFP (green). Tumor cells turn red after the T-cell attacks and kills them (propidium iodide staining). The time-lapse imaging was performed using Nikonâs BioStation. what weâre going to start at work next year!Â
Brother?
âŚnot quite.
Who am I speaking to, if you donât mind me asking?
Only good things I hope!
Yeah, I figured it was something like that. It would be kind of funny if he disappeared because you got here.
Now Iâm curious- You have to tell me everything about everything!
In a sense. I heard Michael getting frustrated about your lack of professionalism, so really, thatâs more or less the same thing, isnât it?
I should hope he didnât. There are a lot of people worried about him.
Ah, itâs not so different it warrants a âtell me everythingâ. *chuckles* Mostly itâs less⌠military in nature. Less strict. The guards at the gates had me baffled when I first saw them.
Ah, Michael. I'm sure he loves us...somewhere deep, deeeeep down.
Well there is not a lot that can take on an Archangel of the Lorde (TM), so I guess we don't have to worry too much. I just hope he remembers to bring back souvenirs. Though it'd be somewhat odd to have two Gabriels - we'd have to give you numbers. Or nicknames.
That sounds...nice? So you guys are not at war anymore?
Brother?
âŚnot quite.
Who am I speaking to, if you donât mind me asking?
I take it you have heard of me then?
Thatâs quite all right really. I guess in a way you are the last person who would know, am I right. After all you are not my brothers keeper. Itâs just that we are a little worried since heâs kind of gone MIA.
So, hey! What is your world like?
Heard of you, though I hadnât had the pleasure of meeting you.
Heâd already disappeared when I found myself here. I wouldnât know where to find him for that reason. I wish I could help, butâŚ
My world, huh? Itâs rather similar to this one, though the people are⌠not so much. Heaven is certainly different from how I remember it.
Only good things I hope!
Yeah, I figured it was something like that. It would be kind of funny if he disappared because you got here.

Hello stranger, then!
I am called Remiel. Have you by any chance seen my brother Gabriel?
Remiel?⌠Ah, right. Of course.
Iâm afraid I havenât seen him, no. Iâm sorry to disappoint you.
I take it you have heard of me then?

Wouldnât dream of it. Never get anotherâs sword out of its sheath without their permission.
Oh you know, blueâŚ.whiteâŚsomething calming
Blue would be decent, yes. It would have to be a dark blue though. Is that not what royalty wears? I am not too sure of human fashion. Although I believe they are particularly drawn to floral patterns.Â
...really?
I believe you mean purple Michael
'âŚok
:D
soâŚ
what would..in your opinionâŚ.make a suitable crown then?â Metal. It needs to be strong and unbreakable, yes? Perhaps you could decorate it with small blue stones.Â
StonesâŚ. yes - sounds lovely. Of course youâd need a cape to go with it.
I was thinking perhaps the metal used could be one of my swords? âŚDo not melt the sword in my sheath. However, I have a large collection so I will choose one from there. Cape? What colour do you think would suit me best?Â

you can tell this is a high ranking bun, because he is wearing a crown that is also a bun
âŚwould this âcrownâ be a bunny orâŚ?
I do not think a bunny would make a good crown, Remiel.Â

Calling on the king of hell
Hell's waiting room, as one might expect, did not lend itself to comfort. The seats were crammed together and made of the exact same kind of plastic that had spawned pretty much each seat in every hospital waiting room ever. A sweaty butt-print was guaranteed and if the poor soul occupying one of these monstrosities as much as thought about moving they squeaked and groaned in hair-raising protest. To round things off bland, brain-numbing muzak was playing in the background and a forgotten, plastic, potted plant was accumulating dust.
The man in the worn jacket who was slouching in his chair, with his legs crossed and his foot bouncing to the beat of the piece of unrecognizable music he was whistling seemed to be blisssfully unaware of this. Instead he was perusing a high gloss magazine he had brought with him, tucked into a pocket while carrying a large, elaboratly wrapped gift basket under one arm. Now he reached out in random intervals to distractedly fuss with the large, lavender coloured bow while his eyes remained fixed on the page. For all intent and purpose he seemed ready to spend the next small eternity right there.
Brother?
âŚnot quite.

Yes. She gets fed up with his attitude sometimes.

Blowing Off Steam
Giving his companion an amused look at her question, as if their work was something they only did from nine to five with a life outside it as the human held it and not the very reason for their existence Remiel shrugged lightly. âNot too bad I suppose.â He mused. âItâs been quiet for the most part.â Really, the divine vision business was barely worth mentioning these days and ferrying the soul of the faithful to heaven only slightly more so. âWhat about you? All this new gadgets and do-dats must keep you quite busy?â
Smiling at the bartender that had finally spotted them he politely requested a brandy then passed the question of choice in beverage on to Liwet.
"You can say that again," Lewie answered with a laugh before ordering her own drink (a pint of old ale). Things had never really been what most people would call "quiet" since sheâd opened her eyes, but it had certainly gotten busier lately. And progress certainly didnât look in any hurry to slow down. "Canât complain, thoâ. Busyâs how I like it. Wouldnât know what tâdo with too much downtime."
She leaned an elbow on the bar, absent-mindedly watching the drink pouring from the tap and into her glass.
ââSides, it keeps things interesting. Didâja know they got a robot the size of a car on Mars?â
"I did notice that." Remiel agreed with a genial smile, though he did not point out that he would be rather hard pressed to say what exactly the thing on mars was or what the humans were hoping to achieve with it. "Mars is the one with the little green men, if I remember correctly?" He accepted his own glass of honey gold liquid with a nod of thanks to the barkeep. "I can never quite keep them straight."
Looking around it wasn't hard to spot an empty table since at this hour only the most hardcore drinkers had already taken up their regular positions. Inclining his head in the direction of the tabe he had chosen he waited until Liwet had received her own drink before crossing the bar room and taking a seat. "I hope I am not keeping you from anything too important."
