Concept: a version of Havelock Vetinari with some kind of lifelong chronic pain condition. Hear me out.
A lot of elements of Vetinari's character could be explained and actually made more interesting by chronic illness. At least in my experience, living with chronic pain forces you to develop a degree of control over your body (e.g. not freaking people out around you by visibly being in pain all the time through facial expressions / voice tone), it can give you a ridiculous pain tolerance, and for some people puts you in a position where you end up with a strong internal fortress which is relatively unshakeable by inconveniences of the physical world, similar to Stoicism in its original ancient philosophical sense -- even when your body is completely going to shit with a million symptoms, you are still Yourself, and you remain rational.
Basically, in short, some people react to chronic pain by becoming a rock in almost all situations.
Now, Vetinari is a man who can be calm when being imprisoned and theoretically having his life in danger; he is able to remain rational and make reasonable decisions when having literally just been shot; and he is able to tolerate the discomfort of being poisoned whilst not allowing the inability to care for himself to affect his internal sense of self. His reactions could for sure be the result of years of training and immense self-discipline (which is the textual implication), but I think it adds a layer of something interesting if he has built up a tolerance to physical and situational discomfort through being in agony for the majority of his life. If this is the case, then I think it's really cool that his disability would actually make him a better Patrician.
As an added bonus, I think this interpretation gets extra fun when you bring in the fact that he's very pale and skinny (pain and other chronic symptoms can affect appetite, and the paleness is kind of self-explanatory), and the fact that he doesn't sleep much (pain-somnia).
Idk. I find it fun to imagine that not only is he resilient and level-headed in the face of crisis because that's the kind of person he is and has trained himself to be, but that he has been forced to become that way by a body which doesn't give him any other choice, other than to simply give up.

I’ve spun my wheel of characters and landed on Twoflower, who is a discworld character , and my favourite thing is that he sees people’s reaction to money and presumably sees other people using money, he does not know how ridiculously rich he is by everyone else’s standards
he is a millionaire who thinks he’s a low payed working class tourist from Bes Pelargic and is quite funny

It occurred to me today that if Terry Pratchett was still around, we'd have a brilliant book about AI by now.
They would build a new machine standing next to HEX and HEX would be... not jealous, of course, why would you think a machine would be jealous? But still HEX would randomly start responding to someone who would spend more time with that new machine.
I'm 100% positive that Dibbler would be mass-printing generated bad romance novels and selling them for profit. All of them would have titles like "A Necklace of Pearls and Flowers" or "A Menace of Dragons and Swords", depending on the audience.
People would buy dis-organisers with new features and the imps would give VASTLY inaccurate answers ("No, dragons don't breathe fire, this is a myth, you are safe!") that would put them in danger - and yet would still refuse to get rid of them. (Maybe Dibbler would sell those advanced dis-organisers, too, sounds like something for him.)
The Watch would have their hands full saving people from said danger. Vimes would never trust this new invention and would get extremely angry at anyone suggesting he bought one, too.
Dwarves would be very anxious because, as our dear Blackboard Monitor already knows, Words Are Important. And if those words come from gods know where, that's really bad. I can actually see the plot where they can actually team up with Blackboard Monitor to destroy those "machine" words at the end.
All that generated rubbish would put L-Space in the danger of collapsing. Because there are books that no one should have written.

this came to me in a vision
if i couldn't have any of the obvious first choice superpowers i would want my power to be the power of i can go anywhere and its not a problem. the way this power would work is that as long as im peaceful and respectful i can go anywhere and nobody will see it as weird or see me as a target for hostility. i can walk into a cool house to politely peep the decor or walk into a tall office building to politely peep the decor or walk around a city at night alone to politely peep the decor or walk into a strange facility to politely peep the decor or walk onto a plane and ride the plane to its destination and then walk around there to politely peep the decor and its fine as long as im chill. no need to make me faster or anything i can just walk at a normal pace. its good for the soul.
