Sam couldn’t understand why Dean needed to speak to him in the infirmary. They shared a room, for God’s sake. Why did he have to walk all the way to the other side of the Rebels’ settlement in order to talk to his brother? In his state, he didn’t really consider that Dean could be hurt. He also didn’t even come close to suspecting his older brother’s true motive.
When Sam walked into the infirmary, he immediately felt something was off. Dean was the first person he saw–and he looked worried. No…guilty.
“Hey, Sammy,” he said nervously, obviously hiding something in his hands. Before Sam was able to prepare himself, both Scott and Isaac came from behind him and held his arms behind his back. He let out a surprised grunt, immediately struggling against the two werewolves.
“Dean–what the hell?!” He looked down at the now obvious syringe in Dean’s hands. “Don’t you fucking dare–”
“We’re gonna make you better, I promise,” Dean said, walking forward as Sam continued to curse and struggle.
“No, you’re gonna make me weak again,” Sam argued, only slightly aware of the pain in his arms from Scott and Isaac’s vice-grips. “I don’t want to be!”
“Sam, you’re not yourself and you know it,” Dean replied, uncapping the needle. “I’m sorry but it has to be this way.”
At that moment, Sam’s struggling made Isaac’s hands slip and he managed to elbow the young man in the nose. Isaac stumbled back with a curse while Scott tried to grab Sam’s free arm–but Dean was at his side in an instant, giving his little brother a hard punch to the gut.
"Dean-” Scott started, afraid the older hunter was starting to go too far. But it worked, and Dean had Sam pinned to the ground in no time.
“You’ll seriously thank me for this later,” he said with some difficulty as Sam kept struggling. Then he plunged the needle deep in Sam’s neck, injecting the tranquilizer and rendering the younger man unconscious in seconds.
