Imagine the harbingers that are out Snezhnaya, Traveler, and the sumeru cast receiving the information where Il dottore is immediately replaced by someone and is continuing on researching all of his past projects. Shocked and suspicious how fast they're replacing him and who is capable enough to replace him.

Maybe Reader in the past is ironically a nurse from sumeru, so being given the title 'Il Dottore' isn't quite far. Reader who also researched about humans, how to improve this illness, or that curse only to be discredited and look down upon.

Reader who meets Dottore -Specifically Zandik, the original- when they were researching or banished from sumeru and becomes their assistant. Slowly and surely, you are the face when they conduct expirements. In inazuma tinkering with abysal energy? Don't worry, it's just reader investigating how their 'patients' got sick.

The sumeru cast needing more help, they turn to a peculiar nurse that Mr. Hatguy knows. One who studied irminsul for the past decades to help people with illness.

The Other harbingers knows of Dottore's little helper, Il Infermiere. But have never seen them.

Il Dottore is defeated, but can't help but feel something is wrong. Especially on how fast they've replaced him.

Il infermiere also means keeper (yes i just got it from google) so you can say reader also keeps Dottore's projects and secrets all to themselves.

Then boom his eyes shut and he’s DEAD.

okay but yk how he was basically dragged down to hell by his own segments, what if his beloved y/n was someone that he experimented on (to make them immortal ofc) but something went catastrophically wrong, and as a result, his beloved is one of those pulling him down as well

so it was his own fault that he let his beloved down, even further, as well as himself
