Well since the pox is a product of LeChuck and Elaine was stricken by it, Guybrush worries that his attraction to his wife might hint towards LeChuck also. A shiver of discomfort would run down any pirate's spine for that thought. I think Guybrush's mind in that instance was worrying about the wrong thing anyways, since hiy attraction was mostly aimed at Elaine appearing different from normal (or he might worry abt. it because he himself was afflicted with the pox at one point)
