To determine your eligibility for our community inclusion services and so we can better address your needs, you must undergo assessment via our NJ Comprehensive Assessment Tool (NJ CAT) and the Person-Centered Planning Tool (PCPT).

We understand how difficult scheduling your loved one's Individual Support service can be when you also have your career and other matters to attend to. Thus, we offer hourly rates and daily rates so you can still avail of this service.

Support brokerage usually covers the following services: a. Coordinating and/or arranging patient’s daily activities b. You can avail of our respite care services for you and your loved one in your home, in a DHS licensed group home, or other DHS community-based setting. Other locations you request will be subject to approval as per State guidelines.

It’s a fallacy that the aged and ill must not do physical activity to prevent further complications. However, physical activity that is safe for your condition boosts the immune system, lowers blood pressure, relieves anxiety, improves sleep quality and heart health, and improves one’s strength and stamina. Safe physical activities are light to moderate exercises, but be sure to consult your physician first.
