Because that's? The joke his character is built around? He's a manchild in design, personality, and behavior. That's kind of the whole point.
His big forehead, thin & patchy hair, large eyes, round face, short and fat body; they're all the features of an infant put onto an adult man so he feels uncanny and subconsciously reminds you of a baby.
He's comedically petty, immature, and defensive. You laugh with him because of his needless vulgarity ---
--and you laugh AT him because of his needless vulgarity. He's a child who just discovered swear words and thinks they make him look cool.
Even the people in-universe who were around him during his teenage years recognized this: The nickname "il bambino" means "the child". He's upset because people are calling him out for being short, whiney, and immature. It threatens the image he wants to project of a mean, scary gangster. He doesn't realize that people aren't afraid of him because hes a gangster, they're afraid of him because he's a violent sex freak who doesn't know how to keep it in his pants.
I'm sure I don't need to spell out how his fetishes tie into him being a manchild, too. He's a grown man who acts like a child and wants to be treated like one, too, especially by women. -1000000/10. Don't let this man within your state lines.
