The thing that's so wild about Chakotay's medicine bundle isn't just that Janeway is the first person he's ever shown it to, even though he's in his early forties(admittedly we don't know exactly when he created it, but I'd be surprised if it was at all recently), it's also the fact that it seems to be the only thing he took the time to save before he sacrificed his ship. Meaning that it's his most important possession. And Janeway is the only person he wants to share it with.
