Okay but hear me out.
Yuuji’s grandfather died of age and sickness with no one but his Grandson at his side.
What if Wasuke Itadori was one of the Hibakusha, bomb affected people who survived Hiroshima and Nagasaki. It could explain his lack of companions outside of Yuuji and deceased family members. And it never specified what his sickness was just that he'd been sick for a long while.
Which would also beg the question, how would Yuuji be affected as being a direct next of kin of a survivor, like health wise?
What if Sukuna makes himself at home, right, settles in. And sees that Yuuji is sick. Like really sick. And it's because of his "king of poisons" affinity that is keeping him in a state of good health.
