Okay, now to your question! I've been itching to answer this, haha!
So in Vadramia I wanted to make transition easier and more fluid. Like, what's the point of having a fantasy world if being trans is difficult?
There are two ways it can work, depending on resources and personal wants, etc. The first way to transition is via a three day course of potions, and this is the cheapest way and similar to hormone treatment. It's also the longest to recover from, as it causes a three to four week fever, not unlike a severe bout of the flu. The potions are the most accessible, and therefore much more common.
For the equivalent of top and bottom surgery though, you would need to go to a magic user, and that's where the money is. Magic users are rare in Vadramia, and there are different types of magic. Nedra, for example, doesn't have any healing magic, so she wouldn't have been able to cast on herself. She had to find a magic user with healing powers, of which there are about ten on the entire continent. That makes travel the most expensive part.
The spell is relatively quick and painless, but leaves "spell-scars" which I sort of envision as something almost like frost on glass, distinctive and beautiful.
I will cover this more in the story proper (they were kind of in the middle of something when Silas found out, haha) but in Nedra's case she went through the course of potions first, then when she had enough gold she found a magic healer. This is a fairly common way of doing things for most people, and was a bit more easy for her as she was travelling a lot anyway.
