I think folk who don't like it read "Andrew never smiles/laughs" as "Andrew is never happy" - but 'happy' is not what laughing and smiling mean for Andrew. In the context of the story, Andrew laughing is a symbol of his medicated state. It represents how Andrew's life is completely at the mercy of others - first his body out of control, and now also his mind, his expressions, his reactions. It's a symbol of how everyone, his family included, preferred to have him drugged to high heaven rather than face the raw reality of the person that he is.
There is not some secret cheerful sunny boy hidden under that black hoodie and those armbands, just waiting for prince charming to come along and wake him up with a kiss.
Andrew - the person that he actually is - is quiet. He's someone who guards his words carefully, who expressed himself with action rather than in words or through facial expressions. When he does communicate it's via silences and cryptic comments that take work and love to understand.
Neil sees him for who he is and loves him because of it, not in spite of it. That Andrew doesn't laugh or smile with Neil doesn't mean that he's never happy, it means that he's found someone who sees and understands him, someone - finally - who speaks his language. Someone who knows that his silence means "this isn't nothing, this is everything", that his quirked eyebrow means amusement, that the glint on his eye means happiness, that shoving Neil's face away means that he's feeling so much he can't stand to look at it.
That's real recovery. It's self acceptance and the safety to not have to perform your emotions in a "normal" or "typical" way, because the people who matter understand you just fine and love you for who you are.
