Vitrectomy surgery in an eye with keratoprosthesis— Artificial corneas are implanted usually as procedures of last resort in eyes with severe corneal stem cell and surface disease. As a retina surgeon I encounter keratoprosthesis in three scenarios: During primary implantation to assist in cases with coexisting posterior segment disease; To repair secondary problems after implantation, like a late retinal detachment; and in the case of infectious endophthalmitis, since the wound never truly integrates into the implant and bacteria can get access into the eye through it. I find these surgeries particularly challenging technically, but very rewarding when blind patients can regain their vision and their independence.
