Dean was not crazy. It did not matter that no one else saw the things that he did, and that he’d learned fast as a kid not to mention the monsters unless he wanted to see his mom tear up and his dad’s mouth tighten with worry and fear. It didn’t even matter that he realized the things he saw had to be the only secret he kept from Sammy, after his baby brother had run to their parents because Dean had terrified him one night stopping a shtriga from killing him.
He did regret that one--not that he’d killed the thing, but that Sam had woken up thinking that Dean was brandishing the knife at him, not the soul-stealing creature about to feed on him.
But despite being not crazy, Dean couldn’t convince people completely...and by the time he was fourteen, he’d had more shrinks than teachers. Or friends. Because people turned out to be monsters way too often, and Dean could not hide what he knew when he was the only person able to see and fucking fight the things that went bump in the dark.
So in the end, Dean was sad but not surprised when his parents agreed with the most recent therapist and had him committed indefinitely at Osawatomie State Hospital shortly after he turned fifteen. He should’ve been starting high school; instead, Dean found himself assessed, given plain blue pants and a white shirt, and assigned to room with some kid named Kevin--he’d had a mental breakdown about academic pressures, and hadn’t really been the same since, apparently--in the hospital’s long-term care ward.
He wasn’t sure if he’d been here one year, or two at this point. Dean knew he was somewhere near seventeen, but that didn’t mean he’d passed twelve months in yet. Time passed differently within the bland off-white walls here.
As I continue down the hallway I pull out the note and saw something else on the sticky note.
Our Secret xxx-xxx-xxxx Dean
A shudder went down my neck looking at the small sticky note with those 10 numbers. Why did he give it to me, I just met him he knows nothing about me. I shook my heading clearing my thoughts as I ripped the sticky note where the number is and put it in my pocket. Entering the class I quickly passed the note to her.
She gave me a quick smile before returning to her lecture as I quickly rushed to my seat. The class went by with ease our professor gave us an essay and work and sent us on our way with a smile.
Since I was finished with all my class, I made my way to the parking lot where Claire and Kaia were waiting.
“Took you long enough I was going to leave,” Claire smirked starting up the jeep. I stuck my tongue out and hopped in the back. As we started pulling out I felt a feeling I was being watched. I turned around in my seat and look out across the parking lot and saw no one. Weird.
As we headed to Claire’s as we were staying for dinner with her mom Jody and older sister Alex. Claire and Kaia chatted away in the front while I was left with my thoughts, feeling the sticky note in my pocket I pulled it out and stared at it. ‘Should I? This is probably a bad idea. Y/n calm down it just your teacher's number.’ Grabbing my phone I put in the number and named the contact ‘D. Winchester’
We pulled up to her house and climbed out and headed to the door and inside. As we walked in I saw Jody sitting at the dining table reading files for a case she was on. Always having important things as Sheriff, “Hey Jody.” Kaia and I said as we kicked off our shoes.
“Hey girls, Dinner will be ready soon and Claire tell Alex that someone was calling her on the house phone.” Jody greeted looking up giving a smile before looking back at the files.
“Sure thing mom.” Claire responding as we headed upstairs to her room. As we passed Alex’s room Claire banged on the door “Alex you’re boyfriend is calling you on the house phone.” She yelled to get a ‘thanks’ yelled back through the door.
We entered Claire’s room as she flopped onto the bed I laughed at her childish antics. Kaia hopped onto the bed cause Claire to roll over. I sat at her desk rolling over to her bed and putting my feet on the bed.
“Claire why were you so mad at De-Mr.Winchester for being in our class.” I questioned as I never got an answer from this morning Claire looked up at the ceiling and click her tongue.
“When I told Dean and Sam I got into this college. Sam was happy for me since he works there and it was a good school. Dean, on the other hand, said I wouldn’t get far and I would get kick out,” She said a small frown on her face. “Words were shouted and I said ‘You watch me succeed and I will rub it in your smug face.” she huffed Kaia grabbed her hand a gave it a squeeze.
“Well after that I haven’t seen him until now no text nothing we both drop off the radar that’s why I’d stay away from Dean Winchester” Claire sighed not wanting to remember that memory. ‘But enough with that crap I’m 22 in my second year in this school with this scholarship that I earned and two of my best friends” She smiled.
I smiled and pulled her into a hug Kaia joining in. Claire groaned pushing us away with a small smile “Gross.” she gagged as we both laughed. As I laughter settled we heard Jody yell for us to come downstairs, Claire and Kaia raced downstairs while I stayed behind. I pulled out my phone looking at the contact ‘Claire has a rough relationship with him but should that dictate what happens’ I sigh I’ll worry about it later.
The sound of my alarm woke me from my dreams. “5 more minutes” I mumble the alarm continued to go off. I groaned and rolled out of bed shutting off my alarm. Getting ready and grabbing an apple and coffee I headed out just on time for Claire and Kaia.
The door that read Law opened before I could grab the handle and out walked Dean. Bumping into him slightly I almost spilled my coffee.
“Woah!” he yelped as I jumped back he grabbed my waist. We stared at each other not realizing how close we were to each other. “Hey,” I whispered he stared back at me. I pulled back and adjust my bag strap.
“Sorry, Mr. Winchester.” I apologized he sent me a small smile. “it’s alright Y/n.” he grinned placing his hand on my shoulder. I shivered at his touch not noticing how his eyes darken. I pulled back and walked around him heading into Sam’s class.
Sam looked up from his desk seeing me walk in as I toss the apple I grabbed before leaving. “Morning Sammy,” I said playful seeing him pull a face here that nickname.
“Y/n just because I let you call me Sam doesn’t mean you can call me Sammy.” He joked around ruffling up my hair. I laughed pushing his hand away trying to fix my mess of hair, he let out a laugh as I try to fix the mess. As we calmed down we heard someone clear there throat. Looking towards the door to see Dean looking extremely pissed.
“Sam a word.” he gritted out of his clench teeth.
Sam tensed and sent me a small frown I looked over a Dean frowning as he avoided my gaze. Walking over to my seat as more students walk in and Sam returns avoiding my gaze his mood dampen. I sighed and let the class go on.
The bell rang dismissing us I got up quickly hoping to ask Sam what's wrong but he walked quickly to his office and closed the door. I let a breath of frustration and headed out. I headed over to the cafe for lunch when I heard a faint yell of my name.
Turning around to see a lanky boy run up to me holding my phone in his hand. “Hey you left your phone in Sam’s class,” he said slightly out of breath giving me a small smile. “Thanks...” I hesitated not knowing his name.
“Jack... Jack Kline,” he responded handing over my phone. I gave him a smile, he shoved his hands in his pockets and rocked back on his heels. “Hey, do you wanna get lunch together as a thank you,” I asked tucking a piece of hair behind my ear slightly blushing.
“Yeah... definitely,” he answered a grin on his face I nodded and grabbed his hand and started walking to the cafe. Not knowing a man in the shadows watching the entire interaction.
“No..no then Claire looked me dead in the eyes and said ‘there only one sheriff in this town’ before passing out,” I explained as we both laughed. Jack wiped tears from his eyes from laughing too hard and I struggled to breathe from laughing as well.
“I’m not surprised Claire did that you and Kaia are the voice of reasoning in her life.” He added causing me to giggle my face turning pink. I sighed as we walked in silence only to be interrupted by my phone buzzing. Claire- ‘Hey where r u we’re waiting’ I groaned completely forgetting the time as I was distracted by Jack
“Crap the girls are waiting for me I didn’t even notice the time,” I said looking at the clock on my phone. “Same I had fun.” He said with a boyish grin. I blushed staring at him.
“Y/n waited!” he yelled while I was walking away. I turned around as he grabbed my wrist “I really liked to be able to hang out again.” he said slightly nervous. “Jack Kline, are you asking for my number? I said smirking a bit.
“If I was is it working?” he flirted back. Giving a boyish grin that can make any girl swoon. “yes it may have just worked.” I replied taking his phone and putting my number in. I smiled at.
“Bye Jack.” I waved starting to walk away “Bye Y/n” he responded giving a killer smile.
When I walked out to the parking lot I saw Claire and Kaia waiting. “Finally what took you so long?” Claire whined wanting to get home, I smiled looking at my phone to see I got a text xxx- Hey its jack I had fun “Hey y/n are you listening at al-why are you smiling at your phone?” She said with a smirk.
“Nothing,” I responding quickly Claire hummed will I climb in only to get a feeling someone was looking at me. Taking a quick glance only to see no one.
As we pulled out I didn’t see someone coming out the shadows “You'll be mine.” They said as they walked to there car and drove off.

“Shh you gonna wake her up.” someone whispered the sound of shuffling slowly pulls her out her dream. Slowly opening up my eyes I jump seeing Claire extremely close to me and Kaia recording in the background.
“Happy birthday bitch!!” Claire screamed in my ear causing me to push her off the bed. Claire yelped a thump being heard. Kaia laughed and so did my parents, I pushed myself onto my elbows to see Claire on the ground Kaia recording and my parents holding a cupcake with a single candle.
“Happy birthday sweetie.” my mom smiled holding in front of me while my dad ruffled my hair. Looking at everyone I closed my eyes making a wish, I opened my eyes and blew the candle out. They cheered smiles on everyone face, “You know my birthday isn’t until this Friday.” I explained.
“Well, honey with your birthday on New Year's Eve we having a small celebration today because your father and I are flying out to have New Years with the rest of the family, while we know you're going to be celebrating with Claire and Kaia.” my mother responded. I nodded with Claire grabbed the cupcake out of my hand and ate it.
I sent her a playful glare as she stuck her tongue out. “yep well be having the time of your life at the bar and parties!” Claire rattled on forgetting for a second my parents were there. “I mean a supervised drink of very small amounts then straight home.” she corrected
My dad gave her a stern look “Just because you’ll be twenty-one in a few days doesn’t be you can drink an entire liquor store.” He stated Clarie nodded. When your dad is an ex-marine you don’t want to get on his bad side. “Ok,” mom clapped her hands,” let's leave them you have classes in a few we’ll see you for dinner tonight.” she smiled kissing my forehead and pull my father out the room right after he ruffled my hair.
“Come on up up get dressed we have classes in,” she checked her watch, “Shit 30 minutes,” she exclaimed. Hearing the time I shot out of bed and rushed to tame my hair while brushing my teeth. Kaia rushed downstairs to make me a quick coffee while Claire started up the car. I threw on a simple outfit and grabbed my bag and phone heading downstairs, as Kaia was at the door waiting for me.
“Hurry up class is in 15 minutes!” Caire honked the horn. Kaia passed me my coffee as we hopped into the car and Claire sped out of the driveway. “I swear if my mom sees me I’m dead.” She mumbled definitely driving over the speed limit.
We pulled up to the school with ten minutes to spare. Waving goodbye to Claire and Kaia as they headed off to their classes while I the opposite. Most of the school day went by a blur. Now I only had Dean’s class before the beginning of break entering his class with Dean not there yet but other’s waiting talking of going over notes.
Feeling a buzz in my pocket I’ll check it Jack- ‘heard your birthday was coming up so happy early birthday’. I smiled sending thanks right as Dean walked into the class “morning class.” he said everyone replied with a greeting as he sat at his desk doing a quick roll call. As he went through the list he looked up at me.
“Y/n happy early birthday,” he said giving me a wink that could make girls and guys swoon. I blushed “Thanks, Professor.” I replied staring back we seemed to stare at each other for a while before he looked away continued roll call. I ran my hand through my hair. The relationship me and Dean was weird but I had no idea where it will go.
Class went by pretty fast Dean giving us work to do over the break. Dean dismissed the class “Y/n can you stay behind for a minute.” he said I looked up from putting my notes away nodded and made my way over to him as the last student left I leaned against one of the desks in the front of the class. Thinking of all the reason why he called me after class.
“Hey, Mr.Winchester, if this is about the recent essay I turned I did try with the resource, is it because I don’t have a work cited I can type it up.” I rambled on trying to figure it out. “No no it’s has nothing to do with your essay it was perfect.” he calmed me down.
I nodded feeling slightly relieved but now I wonder what he wanted. “So, if it’s not that what did you need from me?” I questioned fiddling with hands a nervous tick I had. Dean walked over to his bag next to his desk and pulled a small box with a box on it.
“You’re weren’t going to be at school for your birthday so here.” He passed to gift to my hands, “Mr.Winchester I can’t possibly take this-.” I stuttered shocked
“Yes you can and please call my Dean.” He put his hands in his pockets “Well..” he gestured to the box. I nodded and opened it to find a gold chain with a single pearl at the bottom.
“Dean...I can’t take this,.” I stressed holding the necklace “You can think of it as as a start to the new year gift.” He encouraged giving me a smile that I couldn't refuse.
“Can you put it on?” I asked holding the necklace out to him. Dean smiled grabbing it as I turned around pushing the hair from my neck. His hands went over my shoulder clasping the necklace Letting my hair go he turned me around. The pearl resting on the ident between my collarbones looking up Dean was staring at me.
A piece of hair fell in front of my face bringing my hand to brush it back Dean beat my pushing the strand behind my ear. “It looks good on you.” He whispered I smiled slowly getting drawn into the green in his eyes. Slowly without realizing we both began to lean in.
My nose against his my eyes fluttered closed as he did the same right before our lips met a sharp knock on the door drew me out of a trance and looking towards it. Dean pulled away walking towards the door opening it,
“Can I help you.” Dean blocking most of the doorway from my view I wasn’t to see was there. “Hey! I was looking for a friend of my this was her last class. Her name is-.” he rambled before he saw me “Y/n!” he exclaimed walking into the class without Dean’s permission earning a glare towards him.
“We have been looking for you for so long. We gotta go, remember dinner with your parents.” He said grabbing my bag pulling it over his shoulder before leading me towards to the door. I looked at Dean his face dark sending glares at Jack.
“Ugh, thanks for the gift, Mr.Winchester.” I sent him a smile before walking out the door with Jack.
Dean’s POV
Walking out of my class to see Y/n walk away with this jack hate courses through my veins seeing him grab her hand as she gave him a smile that could light up the room. Hearing her laugh sent shivers down my spine. If that kid didn’t walk in he probably would have kissed her. ‘You’ll be mine Y/n I promise you that if that means removing Jack from the picture I will.’

The sun shone through the curtains. The light blinded my eyes when I opened them, I groaned pulling the covers over my face.
"Y/N time to get up, it's time for school." my mother said through the door before going downstairs.
I stumbled out of my bed taking my phone to my closet, looking through my clothes making a pile. I quickly changed for school, looking at my phone seeing I had about 25 mins until Claire comes to pick me up. I grab my messenger bag and raced down the stairs vaulting myself over the couch into the kitchen grabbing a bar and a fruit shake that I pre-made last night. Sitting on one of the bar stools sipping the drink as I scrolling through Instagram looking at funny pictures of me and my best friends, Claire and Kaia.
A honk rang from outside. I grabbed my bag, my phone, house keys, and walked out the door. I hopped down the steps and saw Claire's Jeep parked in front of the driver’s and Kaia in the passenger. I climbed in and shut the door.
"Hey, loser." Claire greeted and started the car. "Hey did you hear Mr. Henriksen retired, finally that old man always complained about teaching wasn’t his ‘calling’ or whatever," Kaia giggled as we drove. 'Damn that man has been teaching Social Studies for 42 years now' I thought as we got closer to school.
We pulled up at the school we all climbed out, Claire locking her car. Most of the students were outside talking or waiting for an excuse to not be in school. The bell rang signaling school was starting, we walked up to the steps as other students trudged to their lockers to start the beginning of a long year. Kaia waved goodbye as she headed to Ethics.
Claire and I headed over to World History the classroom was filled as everyone waited for the new teacher to arrive. We walked over to our seats that were close to the back as the bell rang. Students gossip or were talking to their peers near them. When we heard heavy footsteps getting closer to the room, the classroom turned to silence.
A man with short dirty blonde hair, apple green eyes walked into the classroom. He was wearing a light blue button-up that was tucked into his slacks that were pair with dress shoes. He held a black briefcase in his hand as he stalked into the classroom looking around at the students then landed eyes on me. It looked liked he stopped for a second and just scanned my figure.
The look in his eye gave off a dangerous vibe. I shook my head clearing my thoughts, he placed the briefcase he was holding on the desk in the front of the room. he walked over to the whiteboard and wrote in an expo ‘Mr. Winchester’. I frowned in confusion wasn’t there already a Winchester he looked over the debate team and taught a class about Law. He faced the class and introduced himself.
“Hello students my name is Dean Winchester but you will call me Mr. Winchester or Professor.” He continued talking but all I could notice was Claire glaring at Mr. Winchester and if looks could kill he’d be ripped in half. The class flew by but I couldn’t help but notice that Mr.Winchester would always look at me and when I looked back he would change his gaze.
When he dismissed us Claire immediately before everyone could put their stuff away flipped him off and walked out. Everyone frozen in shock I shoved my books in my bag and quickly followed after Claire.
“I’m sorry for.. um.. Claire.” I rambled trying to form a proper sentence.
He sent me a quick smile and nodded towards the door. “You should probably after her.” He suggested. I nodded “Yeah bye Mr. Winchester.” I waved walking to the door. “Have a good day...” Crap I never told him my name “Y/N” I responded before walking out.
The door closed behind me just as students began to file out of class. I was able to catch a glimpse of Claire rounding the corner. Catching up to her I grabbed her arm stopping her from moving
“What the hell was that Claire we just started College and you already caused a teacher to hate you,” I exclaimed. She pulled out her phone and began typing into her phone, “Are you even listening to me! You’re putting your scholarship on the line!” I yelled at her.
Claire looked up at me and began to pull me down the hallways and ending up walking with Kaia. ‘He’s not going to hate me because I’ve known him for most of my life.” She explained opening up one of the doors that said, S. Winchester.
Following her into the office Mr. Winchester or as he made us call him Sam as he was more comfortable with it was typing on his computer when he saw us walk in and closed it. Gesturing to the couch he had in his room he got up and walked in front of his desk leaning against it.
“Claire what is it now I gave that key for emergencies.” He asked grabbing his drink as taking a sip.
“Why is Dean and teaching my class Jody said he was going to be here this year but teaching my class is a no-no.” She grunted crossing her arms. I looked over at Kaia who just shrugged her shoulders.
‘I did know that Dean was going to start to work here but him teaching your class I didn’t Principal Shurley said it would be good for the students to have a new teacher and Mr. Henriksen retirement made it easier. There is nothing I can do Claire.” He stated a small frown on his face.
Claire nodded ‘Just because he’s there doesn’t mean I’m gonna like it.” she grumbled.
I laughed lightly and look down at my phone and notice I have a class in a few minutes.
“Shit! I have Latin in a few minutes.” I gasped quickly grabbing my stuff. “I’ll meet you guys at the parking lot later,” I started to Kaia and Claire who both nodded in agreement.
“Later girls, Sam.” I sent a wave and rushed out the door. I speed-walked down the hallways. ‘Why did it have to be on the other side of the building. Not paying attention I ran straight into a body falling back only to be caught. looking up at my savior to see it was Dean.
“Easy there you ok.” He steadies me as I clutch my bag so nothing fell out. I nodded “Thanks, Mr. Winchester.” I said looking at my phone I knew I was going to be late. “Just great I’m late on my first year,” I mumbled crouching down to grab the papers that fell out of my hand.
“Hey Y/N I can write you a pass you seem like a hard-working kid and It is the first day.” He suggested. Looking up at him still on the floor as he knelt down to help “Really I don’t want to be any trouble.” I replied.
“I insist really it be our little secret” he urged adding a small wink. I chuckled causes his smile to grow wide. He pulled a small sticky note from his pocket and scribbled down his signature and holding it out for me.
Going to grab it he pulled back “Our secret?” he questioned. It was kinda strange since it was a teacher helping out a student but I shook it off. “Our secret,” I repeated. He let me grab it our fingers grazed sending a shock up my hand.
Giving him a small smile I started to walk off “Thanks again, Mr. Winchester.” I said. He shoved his hands in his pocket “Call me Dean.” he responded with a smile.
I nodded waving not knowing he didn’t leave and stayed they cherishing the moment we had.
As I continue down the hallway I pull out the note and saw something else on the sticky note.

This is great, just a way to get little plots out of my brain. Don’t hate me for this, please?
It’s become a routine, although he never thought it would, it has, just like taking his courses at Stanford and making dinner on Thursdays, when Jess has the late shift at the hospital.
Showing his ID (sometimes he doesn’t have to anymore, a few attendants take pity on him) and being led down countless corridors to where his brother is being held (lives, lives, he should get used to it) has become a routine.
He remembers when it hurt far more than he does now, the beginning in another hospital when he still had hope, and he can’t say if the dull acceptance that has been steadily building up in him is better or worse than the continual torment of those days.
Today, Dean is happy to see him, and thankfully reasonably happy, not like last week when he greated him tearfully and then poured salt he’d kept from lunch over him to make sure he was not a demon, then proceeded to yell how glad he was Sam made it “out of the pit”.
Apparently, Sam had gone to Hell the week before, the doctors informed him.
Today, Dean is just happy, and it eases something of the knot that seems to have made a permanent habitat in Sam’s throat when it comes to his brother.
It’s always there, whether he’s seeing, talking or thinking about him. In the first few weeks and months, he had to talk about him a lot.
Before Bobby’s frantic call, before he rushed down to Lawrence, he was known among his fellow students as clever, dexterous, some even called him a “prodigy”.
Now he’s the one with the crazy brother.
He’s also the one who visist him every week, craving to find traces of the man who raised him, the man he was before his mind snapped and made him believe all Dad had mumbled about when he was drunk.
Sam never should have allowed Dean to solely care for him when he left for Stanford. He should have visited more often after Dad died.
He should have -
He doesn’t know what he should have done, but he would have done everything to spare Dean this fate.
Jess used to accompany him, at the beginning, before the black word “untreatable” was thrown around, before the doctors stopped making Dean take meds, shrugged their shoulders and talked about “permanent accomodation”, as if Dean was looking for a new apartment, not a room with soft walls and barred windows. But Jess can’t visit anymore because Dean freaks out every time he sees her, trying to drag Sam away, insisting that she’s just an illusion of his dead girlfriend “out to get him”.
Why she can’t come back to life like Sam and Dean seem to do on a regular basis, Sam has no idea.
But today, Dean is happy and doesn’t throw salt at him. Today, Dean looks even a little excited (on a normal level, thank God, it feels so good to see him like this).
Soon Sam learns why.
“I found him Sammy, I found him!”
“Who?” he asks, slightly concerned.
Dean rolls his eyes and points to another patient who’s sitting on the table next to him, watching them with bright blue eyes.
For whatever reason, he’s wearing a trench coat over his scrubs.
A memory of one of the craziest stories his brother told him flashes through Sam’s mind, and he knows what Dean is about to say.
“Cas, Sam. Cas came back!”
Castiel, the angel who rescued Dean from Hell when he was dragged there by hellhounds (the first time? the second time? They seem to die a lot and Sam has trouble keeping track of the stories his brother’s mind weaves, they are growing more and more complicated and are all connected).
“Oh?” he inquires because he still can’t bring himself to act like he believes Dean, like he’s part of his fantasy world, not even to make Dean feel better.
Part of him still hasn’t given up hope. He knows it’s ridiculous, but he can’t help it. Somewhere in his mind, there has to be the brother he remembers, the one who fed him and read to him and fixed cars and proudly waved after him as he left for Stanford, not the ghost hunter who never gets to rest.
He’s astonished when the blue-eyed man, who really looks like Dean described him, says, “Hello, Sam”.
And suddenly he’s caught in a conversation with two people who seem to agree upon everything crazy their minds can come up with.
He lasts for an hour before he excuses himself and goes to look for Doctor Crowley, who Dean sometimes likes and sometimes doesn’t because apparently he’s the King of Hell.
“Mister Novak” he confirms. “His wife had him committed when he assured their daughter he wasn’t her father. His delusions complement your brother’s to an extraordinary degree”.
“How is that possible?” Sam demands.
Doctor Crowley sighs. “I don’t know, Mr. Winchester. I can only run this place and watch things take their course”.
And they do.
Over the next few months, Sam watches his brother and the man who thinks he’s an angel fall in love and doesn’t know what to do. There are gentle touches and blushes and long gazes, and he would be happy for Dean if -

The doctors are as clueless as he is.
“We tried keeping them apart” Doctor Crowley admits one day “It led to an almost complete breakdown for both of them,”
“Does his wife -” Sam trails off as he watches Dean whisper something in Cas’ - no, Jimmy’s, he refuses to give in to the madness - ear and the other man giggle.
“She doesn’t even visit. According to her, it would only hurt her and her daughter. It’s not an unusual reaction. If I may say so, you are the exception”.
Sam can’t feel proud at that.
But eventually, he finds something like peace.
Because one day, he walks in on Dean and Cas holding hands and Dean announcing “Cas is human” and they both look as happy as they can be.
I've wondered for a while with this show about the sanity of all the characters. But the most troubling of the lot has always been Dean.

And there have been things along the way that imply it's not out of the realm of possibility for Dean to have lost his mind.
And in that same episode, Sam told him, "Well...come on. I mean, you've been at least...half crazy for a long time, and since you got back from hell, or since before that, even. I mean, we're in a--we're in a mental hospital. Maybe you finally cracked! You know, maybe now you are really...for real...crazy."
He was diagnosed in this episode as a paranoid schizophrenic with narcissistic personality disorder and religious psychosis, while also drawing attention to his alcoholism, poor sleep habits, and impersonal relationships. Except for the dangerous codependency he shares with Sam.
Aside from the obvious points, there have been other subtleties throughout the series that might implicate something that Dean has never outright mentioned. A possibility of occasional hallucinations.
For example, in the end of "Yellow Fever" after Dean is cured of ghost sickness, he sees a flash of yellow in Sam's eyes. This was supposed to represent Dean's fear that Sam would go dark side but he shouldn't have been hallucinating anymore. But he was.
There's also how he deals with hallucinations or similar when they do occur (usually with a supernatural cause). In "Clap Your Hands If You Believe", Dean is abducted by faeries and sees a redcap following him. He points it out to Sam who doesn't see him.
"He's right there- (redcap is gone) Well, now he's gone."
"Who's gone?!"
"Can we please just get out of here?"
"What's wrong with you?"
"Before I hit you?"
Dean brushes it off, not wanting to get into it with Sam which would probably imply this has happened before for reasons he doesn't want to get into with Sam, like, he's hallucinated before.
Dean is frequently asked, "What's wrong with you?" sometimes relating to his low sense of self worth, sometimes relating to his extremely violent tendencies, etc.
