It’s Princes and Peasants: Smallpox in History by Donald R. Hopkins.
Hopkins was one of the people behind the eradication efforts for smallpox in the ‘70s, and he wrote this book shortly after. It doesn’t go into detail about the eradication effort itself- it’s mostly focused on inoculation and vaccination pre-1900- but it’s still a really interesting, if kind of dated, read.
