Jarisch-Herxheimer Reaction

It is a reaction to endotoxins released by death of an organism. It occurs because the release of endotoxins or lipoproteins from organisms is occurring faster than the body can clear them.

It resembles bacterial sepsis. The usual scenario would be, onset of symptoms within a few hours after administering antimicrobial agents such as penicillin or tetracycline for treatment of syphilis (or other microorganisms - spirochetes, borreoliosis).

Signs and Symptoms include:

hypotension

headache 

tachycardia

fever

chill / rigor

hyperventilation

vasodilation (flushing)

myalgia

exacerbation of skin lesions

The intensity of the reaction reflects the severity of the condition.

Management

Prophylaxis and treatment with anti-inflammatory agents

Oral aspirin, prednisolone (steroids generally have no benefits)

Monitor vital signs especially blood pressure

Meptazinol to reduce the severity
