I'M FUCKING CRYING.My dear Carmen just called me from Madrid,she's doing her resident as a doctor in psychiatry,just as she wanted.And it gets better cause she's staying here,she doesn't have to move to another city.TEARS OF JOY

In a few months I'll be choosing a specialty to start my residency and I have doubts between choosing Psychiatry or Family Practice; I like both. Any advice? -entropycomeseasy

When I first started med school, my dean told me "if you like a little bit of everything and you don't hate psych, you should be a family doctor." She's totally right. We see tons of psychiatry in family medicine. And I really enjoy the FM-type psych stuff. I like treating depression and anxiety and cognitive disorders. I am not a big fan of treating acute psychotic breaks and such. Which is why I'm a family doctor and not a psychiatrist.

But if you like it all, why not do both? There are combined Family Medicine-Psych residencies out there. They're rare, but they do exist. I think in the next few years there may be an expansion in primary care-psych programs, especially considering the shortages of each these days. There was an article about these programs on KevinMD recently. 

If you want to choose one or the other, consider the following:

1. How hands-on are you? Will you miss physical diagnosis? 

2. Do you like the idea of spending more than 15 minutes at a time with your patients?

3. What are your absolute must haves in a career? 

4. Do you want to do mostly inpatient, outpatient, or both? Both careers have opportunities for all of these. 

5. Do you like to see immediate results, or you ok with slow progress?

6. How much variety do you hope to have in your future practice?

7. Where do you plan to practice? What is the need there?

8. Why do you like each of these fields? Is it because you had a great rotation, or do you really buy into the philosophy and lifestyle of a certain specialty?

9. Are you ok with doing basic psych and not the complicated stuff?

Oh god,that's a tough one!Probably the 20's or the 40's,I love that decades.Also,fashion was beautiful.

Be able to fall asleep anytime,anywhere.Most of my nights are slepless nights,so that would be pretty nice.
